I. Reason For Referral Clarice Precious Jones is a sixteen year old African American mother of two who has been referred to this agency, The Harlem Hard Times Coalition, by Ms. Weiss a Social Worker at the Department of Human Services. Clarice Precious Jones prefers to be addressed by her middle name Precious. Precious is currently attending school at Each One Teach One while living with her second born child Abdul in a halfway house. Precious is in need of continuing services to help her to become independent while continuing her education and trying to regain custody of her oldest child, Mongo. After interviewing Precious about her life and goals a safety plan has been put in place. The safety plan consists of Precious remaining at the …show more content…
Though it was later found out through interviews with Precious and Ms. Weiss that Mary had committed fraud and did not have custody of Mongo. Previous school administrators, including the principal Ms. Lichtenstein, relayed information about Precious as a student. The administration informed the social worker that once Precious became pregnant for a second time while in high school she was expelled from school and referred to the Each One Teach One education program. Teachers at the school informed the social worker that Precious was an attentive student but had a very low reading level and therefore struggled in school. Ms. Weiss was able to inform the social worker of aspects of Precious’ life including past abuse and relationships with family members. Ms. Weiss was also able to provide the social worker with past welfare records for the Jones family. Ms. Rains was able to provide the social worker with current educational records and more information about Precious’ recent struggles. Hospital staff provided the social worker with medical records and conformation that Precious’ was taught basics of breastfeeding and taking care of a newborn. Nurse John was able to give the social worker information on Precious’ state of mind at the time of birth and during her hospital stay. From the counselors of …show more content…
She is currently receiving welfare checks to help support her and her daughter. Precious feels this economic support is adequate for her currently. Though she plans to get a job to add extra income once she has finished her G.E.D. She is able to meet her needs with this economic support and her halfway house services. However she is not able to save money or have any extra. She is currently enrolled in the Each One Teach One program and is at a 7.8 grade reading level. She will soon graduate from this program and begin her college career. Precious currently lives in her own room in the halfway house with one of her children. Precious feels safe in the halfway house and walking to school through the surrounding neighborhoods.
