The case that has been presented to us addresses the issue of measuring and improving quality through evidence-based quality management. The information in this particular case was obtained within a home health organization. The organization is The Visiting Nurse Association Health Care Partners of Ohio (VNAHPO). VNAHPO is a not-for-profit agency that provides a variety of services including home health services, hospice cars, and private duty nursing. It is a large community healthcare organization located in Ohio that encompasses 6 organizations. VNAHPO prides itself as an agency for their efforts of innovation and community service.
The largest service provided by the corporation is their home health service which generates approximately $30 million per year. Home care is a continuously expanding and diverse industry that is part of the large scale continuum of healthcare services. Currently around 7.6 million individuals receive home care and Medicare is still the largest payer for home care services. Recently concerns about quality of care and the cost of care have stimulated further data collection. Electronic information systems have made the collection process much easier however VNAHPO has been challenged to create an innovative data collection and monitoring method that will significantly help quality improvement.
VNAHPO presently has a quality improvement program that provides an ongoing, planned method for accurately observing customer satisfaction, the quality of patient care, utilization of services, risk management, and efficiency. The current program has the following two major goals: 1. To monitor the processes and outcomes of services, 2. To comply with quality improvement program requirements established by regulatory and accrediting bodies
