Five Pathogenic Care Realms: A Case Study

Due to the client losing both her parents during infancy in a tragic car accident, the client’s disorder can be viewed as a disorder of attachment. The client’s history shows deficits in three of the five Pathogenic Care Realms: settings where there is persistent disregard for the child’s emotional needs, settings in which there are repeated changes in primary caregivers, and settings in which child is raised with limited opportunities for stable attachments (American Psychiatric Association, 2013). The strict, cold religious school where the client spent the majority of her childhood offered little support for the client’s emotional needs. The client did not bond with any teacher at the school and had no particular primary caregiver after the death …show more content…
The client experienced a detachment from her family, not just losing her parents to death, but also being separated from her twin brother.
According to Bowlby, the client’s difficulties in maintaining boundaries, periods of abrupt withdrawal, and subsequent movement through a period of anxious attachment, exemplify steps towards internalization of a protected base of attachment (1988). A toddler looks to the mother as a secure base to return to when frightened. By contrast, the client’s secure base was tragically removed, thus leaving her in a state of despair, waiting for her mother to return. When her mother does not, the client begins to detach. The closest adult figures to a secure base the client has are the teachers at her school. When they turned on the client, exclaiming her as blasphemous, the client dissociates to cope with the overwhelming distress, creating an alternate identity. Freud received his medical degree in 1881and later in 1883 he began to work in Vienna General Hospital. Freud spent three years working in various departments of the hospital and in 1885 he left his post at the hospital to join the University of Vienna as a lecturer in Neuropathology. Following his appointment as a lecturer, he got the opportunity to work under French neurologist Jean Charcot at Salpetriere, the famous Paris hospital for nervous diseases. So far Freud’s work had been entirely concentrated on physical sciences but Charcot’s work, at that time, concentrated more on hysteria and hypnotism. Freud’s studies under Charcot, which centered largely on hysteria, influenced him greatly in channelising his interests to psychopathology. In 1886, Freud established his private practice in Vienna specializing in nervous diseases and soon afterwards got to his fiancée Martha Bernays, who came from a well-known Jewish family in Hamburg. It is a shame that we do not tap this rich source more often in our daily conversation to express ourselves more clearly and precisely. There are of course thesauruses but they mainly list common words. Other vocabulary books list difficult, esoteric words that we quickly forget or feel self-conscious using. However, there is a bounty of choice words between the common and the esoteric that often seem be just on the tip of our tongue. Vocabulary 4000 brings these words to the fore. Whenever possible, one-word definitions are used. Although this makes a definition less precise, it also makes it easier to remember. Many common words appear in the list of words, but with their less common meanings. For example, the common meaning of champion is “winner.” A less common meaning for champion is to support or fight for someone else. (Think of the phrase “to champion a cause.”) This is the meaning that would be used in the list. As you read through the list of words, mark any that you do not know with a check mark.
