Ban visited expectant women at the clinic and planted a tree on its premises. UN Secretary General Mr. Ban Ki Moon praised government’s steps as he visited a village in Moulvibazar, where he comments that rural healthcare in Bangladesh is on the right track to meet the MDG targets of reducing mortality and morbidity of women and children. The UN chief flew to the village from Dhaka on a helicopter along with UNFPA Executive Director, Dr. Babatunde Osotimehin, Senior Secretary of Government of the Bangladesh Mr. Humayun Kabir, UNFPA Country Representative, Mr. Arthur Erken, and other high government officials, and dignitaries. He along with his wife came to Mobarakpur community clinic, while the Chief Whip of the National Parliament Mr. Abdus Shaheed M.P. received the guests at the helipad. “Bangladesh is doing an excellent job in providing healthcare to poor people, especially women and children. I want to showcase your exemplary success stories to other countries,” the UNSG said while talking to a group of mothers in Mobarakpur village, 15 kilometres north-east of Moulvibazar town. When Piyarul Begum gave birth to her baby boy at a community clinic set up on a paddy field at a remote village four months ago, she could hardly imagine meeting the UNSG. Much to their surprise, Piyarul and her fellow villagers saw on 15 November the Mr. Ban Kimoon walking through Mobarakpur. As Ban picked up her son Imran from her lap, she couldn’t believe her eyes.

“Today I am the happiest woman in the world,” said a beaming Piyarul after meeting the UN Chief. She and her son are the symbol of Bangladesh’s progress in healthcare services to women and children. “The GOB and UN will invest more in healthcare services for women, children and youth in the future”, he said.

Joint MNHI improves the health of mothers at Jaborhat

Jaborhat is a remote area about 15 kilometers from Pirganj upazila in Thakurgaon district. Considering the remote location and demand for skilled and institutional delivery, MNHI setup a fully equipped 10 bed maternity unit in the Union Health & Family Welfare Centre (UHFWC) in March 2011. The GOB supplied all essential and emergency medicine for saving lives of mothers and newborns. The UHFWC was given a ‘new look’ to attract pregnant mothers and other clients. Mr Ramesh Chandra Sen, Honourable Minister for Water Resources, who is also Chair of the District MNH Committee, inaugurated the new maternity unit. UNFPA Representative, Mr Arthur Erken was the special guest at the ceremony.

Pregnant mothers of Digpait union of Jamalpur now enjoy free transport services

Expectant women of Digpait union of sadar upazila, Jamalpur now have the opportunity to use a free transport service to access health facilities. Haji AKM Mahbubur Rahman Mohabbat, Union Parishad Chairman, introduced this service specifically for the poor pregnant women of this union. In December 2009 when Mr. Mohabbat attended the MNHI Upazila Coordination Meeting to establish the Community Support System (ComSS), he found that many pregnant women with complications died as they were unable to reach a health facility on time due to lack of transport. This inspired him to donate a three wheeler rickshaw van to the local health authority in January 2010. He also arranged a mobile phone for the van driver and printed the mobile phone number 2

MNHI also carried out capacity building of the staff on antenatal, postnatal care, use of

on the van-body, so that people could easily access the van on demand. Incidentally, a few days later, a pregnant woman with severe complications was saved after she was rushed in the rickshaw van to the district hospital. The initiation of the transport service in Digpait Union is a classic example of engaging local government personnel in the MNHI to save lives of women in their own community.

As Chief Guest, the Senior Secretary of MoHFW pointed out several key issues, such as accessibility, human resources, demand for services and service delivery at different tiers. He informed the audience about changes in the health and family planning programmes under the new health sector programme. He emphasized that the LLP would be one of the approaches to meeting the local health service needs through local participation. He also stressed that facility-based service delivery, which currently is a too low, needs to be increased. He firmly believed that this initiative will benefit most of the people in the district. He then declared open the MNHI LLP in Sirajganj. The Deputy Commissioner of Sirajganj as chair of the inaugural session concluded with hoping for further successes in maternal and neonatal health in Sirajganj. Among others Dr. Md. Syedur Rahman, Programme Manager, Dr. Alim, Deputy Programme Manager and other officials from Line Programme Essential Service Delivery (ESD), Directorate General of Health Services (DGHS) spoke on the occasion. Dr. S.K. Chanda, Technical Officer, MNHI, on behalf of UNFPA and three UN agencies thanked the guests and participants.

Mr. Md. Humayun Kabir, Senior Secretary, Ministry of Health and Family Welfare (MoHFW) inaugurated the MNHI Local Level Planning Workshop for Sirajganj district in Shaheed Captain Monsur Ali Auditorium on 18 October 2011. The Civil Surgeon of Sirajganj in his welcome address thanked the chief guest, and participants. The special guest, Dr. Shamsul Alam, Line Director, regarded MNHI as an opportunity for participation of all stakeholders in the district. He hoped that through MNHI, the EmOC facilities would be strengthened and demand for services would be increased. He also emphasized the need for developing high quality local level plans to guide district-level activities aimed at reducing maternal and neonatal deaths.

Lahirikandha Community Group uses local folk media for MNH awareness campaign

The Community Group (CG) Members of Lahirikandha Community Clinic organized a campaign on maternal and neonatal health issues to increase awareness in the community. District health officials along with local civil administration and representatives of local government attended the programme. Widely renowned folk songs (Gomvira) put focus on the importance of birth planning, safe institutional delivery, child immunization, breast feeding, and spacing pregnancies. Before performing songs, a question and answer session was held, and some pregnant mothers and their family members shared their experiences in receiving MNH services from the facility.

Director Primary Health Care praises MNHI at Thakurgaon

As a part of his extensive field trip to the North-western districts of Bangladesh, Dr. Syed AJM Musa visited MNHI at Vabnaganj Community Clinic and Pirganj UHC Thakurgaon on 6 August 2012. The main objective of his visit was to observe the current MNHI interventions. The Line Director was received by Mr. Ishak Ali, Upazila Chairman of Pirganj, Mr. Ekramul Haque, Chairman of Saidpur Union, and Mr. Wahiduzzaman, President of Vabnaganj Community Group. Dr. AH Tafafder, Civil Surgeon, Thakurgaon, Dr. AMK Kabir, President, BMA of Thakurgaon, and Mr. MT Islam, Deputy Director, Family Planning of Thakurgaon, and Dr. N. Islam, UHFPO. The Line Director was impressed with the vegetable garden initiative by MNHI and community clinic group at Vabnaganj Community Clinic which encouraged pregnant mothers to consume nutritious meals from natural sources. He opined that this clinic should be an example to other clinics in Bangladesh. The Line Director was informed that the vegetable gardening initiatives are now replicated in additional 20 community clinics in the district. 4

Dr. Musa later visited the clinic and critically scrutinized the performance through registers (ANC, PNC and Delivery). He also had a short conversation with Ms. Radha Rani – the Private CSBA trained under MNHI from Kumudini Medical College under PublicPrivate Partnership (PPP), about her performance on safe delivery both at the clinic and household level. He commended Ms. Rani for receiving the Best Performance Award on CSBA from the Honourable Prime Minister on the eve of observance of World Population Day 2012. Also, the Line Director instructed the local health authority to provide an ID card mentioning that the private CSBA was trained by the government.

However, he showed concern over the inadequate number of beds in the obstetric and neonatal wards. Maternal and Perinatal Death Review initiated at Thakurgaon district in 2010, then expanded to other three MNHI districts. Total 306 maternal deaths, 3371 neonatal deaths and 3056 stillbirths were reported in 2011 in 4 MNHI districts. Verbal autopsy was conducted in all cases of maternal and one third of other deaths. The analysis of verbal autopsies and facility death reviews revealed a scenario which would be crucially important and new to the programme managers at district or above/below to understand own status and identify actions to do a change based on ground data. The Line Director also praised the Vabnaganj Community Group for having a ‘revolving fund’ for use by poor women including use of a rickshaw van for referral of pregnant mothers. He also observed community initiatives such as, blood grouping, social mapping of pregnant women according to wealth quintiles, ANC, PNC referral system and work of Community Health Volunteers supported by a local NGO (ESDO) under MNHI Community Support System. Later, he attended a social autopsy session which was carried out by CIPRB under MNHI. The Line Director, Dr. Syed AJM Musa, visited Pirganj Upazila Health Complex. He was impressed with the cleanliness of the ‘shoe free’ OBGYN ward and birth delivery figures,

Death Mapping : A case study
MPDR system notified 4 maternal deaths, 21 neonatal deaths and 15 still births to occur during 2010 in Kashipur union (a population of 22,000 approx) of Ranishankoil upazila under Thakurgaon district. The union came to the notice of the local Upazila health manager during preparing the Death Map of the upazila. The fact was then presented at district MPDR review meeting in presence of Civil Surgeon and DDFP: the two District Managers. Instantly the committee decided to explore the fact and found that the area was remote, with poorer and mostly illiterate people. Verbal autopsies and Social autopsies provided important information to act on. There was one community clinic which was nonfunctional due to logistics and manpower. The authority to decision to deploy one CSBA to work there and Joint MNHI provided all other logistics support in addition and launching awareness activities. The community clinic started working in hands to hand with the community and partner NGOs, Community support system etc. Coverage with ANC, safe delivery and PNC including Newborn care increased. Referral was increased. The actions in collaboration thus gave a positive result within a quick period. The data of 2011 showed a remarkable reduction in deaths: no maternal death, 8 neonatal death and 5 still births reported. Analysis of maternal referrals showed that at least few near miss cases were referred at earliest to district hospital by the community.
