To me accountability is being liable for my actions and responsibility is how I proceed from those actions. As I write this, my minds brings me back to the night I was taken care of 18 year old. I was working on the step down section of the neurology unit. This patient was admitted because of a drug overdose, which left the patient in state like she had a stroke. I remember going to assess her at the beginning of my shift ( 12 hour night) and as I was conducting a neurological assessment, the patient was taking to me or responding to my presence. Which the off going nurse had said in report that the patient wasn’t responding to anyone’s presence. When I assessed her pupils, they were both dialed and not responding to my pen light. Just to double check I gave it another minute and assess her eyes again. So before I did anything else, I notified the on call physician and explained my findings. The physician came to her assess the patient. While assessing the patient the physician said, my report from the previous doctor said that she was showing more response than she is now. When the physician assess her pupils and noted what I had seen, the physician ordered a stat CT scan. Which there was a CT machine located on the unit. So I and the doctor wheeled the patient to the CT suite, where we met the on call radiology tech. As a result, the CT scan showed that the patient had a bleed in her brain. That patient was then transferred to the Neuro ICU department, which was just step away from the CT suit. So in conclusion, I was liable for my actions. My actions was the conduction of the assessment and what I was going to do about my findings. My responsibility was to do something about that action. If I didn’t account for my actions and not showed responsibility, the patient wouldn’t have gotten the care that she needed.
