The article is Enhanced Task-Oriented Training in a Person with Dementia With Lewy Bodies. The purpose of this article is to use a single study participant to show the effectiveness of the STOMP intervention with a person with dementia with Lewy bodies (DLB). The STOMP intervention is skill building through task oriented motor practice which could be used in future occupational therapy treatments. 6. What were the research questions, if any?
The subject was enlisted by a neurologist who specialized in dementia. Mrs. C. was a 73-year-old Caucasian that had many life roles. She was diagnosed with DLM two years ago, but had some symptoms six years beforehand. Mrs. C. never had any therapy directed at her dementia before this intervention. There were a lot of inclusion and exclusion factors. The participant must have been: within a community, speaks English, between the age of 55-90, living with a caregiver, with a diagnosis of DLB, and the ability to complete three hours of intervention every weekday for two weeks. Furthermore, it was important that the client had a Mini-Mental State Examination between the ranges of 10 to 25, which implied mild to moderate dementia. The participant must be able to understand, and follow one-step commands. Moreover, the study needed someone whose family could help choose what goals that the therapy team could assist the participant with throughout the study. The exclusion factors were that the participant could not have Creutzfeldt–Jakob disease, delirium, or severe anxiety. C. COPM goals using the Goal Attainment Scale (GAS). The most important goal, being able to stand from the recliner, did improve by the end of the intervention. On the other hand, the goal of brushing her teeth missed the expected performance level. She complained of lower-back pain which could not be soothed by modalities or medication. The therapist created both a sequenced written and pictorial list; however, her inability to retain her attention on the activity caused great hardships. This goal did not achieve any improvement. The last goal, which was to don her glasses on her face, did improve but not as much as goal one.On putting on her glasses, the final GAS score improved by one level. Some issues came up during the intervention, which could have changed the results of the study. One of the major problems that the therapist and client faced happened to be fatigue. Mrs. C. reported that she did not sleep well eight out of the ten sessions. The decreased activity level caused 20 to 30 minutes each intervention to be focused on resting. This may have skewed the evidence slightly. Furthermore, Mrs. C.’s lower-back pain, during brushing of teeth, required frequent activity breaks, which could link to why the second goal did not see any improvement.
