Human Factors Analysis

...Human Factors Analysis The Colgan Air 3407 accident, was a commercial flight that crashed in Clarence Center, New York on the way to Buffalo, New York. The flight crash was due to poor piloting and background conditions that have changed regulations in the aviation industry. In this essay we will cover this accident using the PEAR model to evaluate the accident. The first aspect we will evaluate of the accident is the people element of PEAR. The people element covers anyone involved or responsible for the accident as well as the factors affecting them. In the accident it was deemed that the captain was responsible for the accident, as the captain didn’t respond properly to the stick shaker which is an indicator that the plane could stall, his failure to monitor the plane's airspeed, as well I think it was the fault of the airline company and how they managed everything. Some of the other factors of the people part of pear is how they treated their pilots, overworking them, making them travel long distances from home which made adequate rest difficult, and just and overall difficult work environment. The next aspect of PEAR is environment, which deals with environmental considerations such as weather, lighting, other personnel and many other factors. In the Colgan air accident environment played the role with the weather conditions primarily. Before the crash the weather caused icing on the plane, and when this was combined with fatigued pilots, one being sick, as well...

...to analyse critically the management of Costa Concordia disaster and how the disaster management impacted on the risks and consequences in the disaster area. This paper also reveals how disaster management influences the management style and process. This paper also recognises lessons that can be learnt to minimise the consequences that occurred. Causes, Triggers and Drivers In a disaster, there are number of causes, triggers and drivers for a specific chaos to happen. In Costa Concordia’s issue the main cause of this was the ship hitting a sandbar with a huge rock on Friday evening near the island of Giglio in Italy (Westcott, 2012). This cause was triggered by the captain of the ship have sailed to close to land as this was an unapproved and unauthorised deviation in course (Johnston, 2012) and the captain confessed that he made a navigational error and gave an order for a turn too late (BBC News, 2013). The drivers for the chaos were the rock that the ship hit was not marked on the nautical chart (Johnston, 2012) or failure of electronic navigation systems. The crew of the ship had also failed to give instructions on how to evacuate the ship due to evacuation drill was not conducted earlier (Westcott, 2012). Pirate Oats will be the first cereal of its kind offering the health benefits of whole grains while still being preferred by the target market of American children under the age of five. Children will favor Pirate Oats because of the cartoon mascot Captain Grain pictured on the front of the box. Exxon Oil Spill

...Summary of Valdez On March 23rd, 1989 a ship called the Exxon Valdez was leaving from Alaska on the way to California. The Valdez was hauling over 53 million gallons of crude oil at the approximately 9:12 PM, the time of departure from the port. The Valdez was piloted by Captain Joseph Hazelwood and Marine Pilot William Murphy. Accompanying the pilots were 19 crew members to assist with the haul. The ship was originally scheduled to depart at 10:00 PM, however, the departure time was changed without notification to 9:00 PM. Members of the crew spent the day relaxing in local bars, shopping, and taking it easy prior to departing the docks. Directly upon their arrival to the Port they realized the ship was loaded and ready to leave for California immediately. The ship was to travel along the Prince William Sound, which was a common oil hauling route during the time of the incident. When the ship was traveling on its route, Pilot Hazelwood decided to go to quarters for a few hours of sleep. The absence of Pilot Hazelwood left Pilot Murphy alone to operate the ship, strictly against company policy. According to policy two pilots are to be on the bridge at all times while the ship is moving. Murphy finally called Hazelwood back to the deck to help pilot the ship after a two hour break. At this time the decision was made to increase speed since the tugboat was no longer towing the ship through the narrow waterway. Captain Hazelwood departed the deck once again leaving another...

The article by Epstein (2011) highlights the global comparisons between Android and several other of the smartphone giants in the industry, and breaks down the comparisons by region. 2. Relevant Vocabulary Year-over-year—results compared year by year for a specific part of the year, as, for example, directly comparing the first quarter of 2010 with the first quarter of 2011. There are no difficult terms to identify. 3. Key Concepts Tactical Planning – Process of defining implementation activities that the firm must carry out to achieve its objectives (Ch. 6, p. 185). Business Strategy Case Study on China Airline

...Executive summary China Airlines Ltd. (CAL) is a Taiwan-based airline. It was founded in 1959. In the early years, it undertook mostly military contract work. It was then declared as the official airline of Taiwan in 1968. Early on, CAL had a lot of problems including poor safety in 1990s which severely discredited the image, faulty pilot recruitment policies, lax maintenance systems, high cost operational structure, inefficient corporate culture, and strained political relations between China and Taiwan which prohibited the airline from launching flights to route in China. On top of that, the economic crisis caused the company huge loss. However, CAL put remarkable efforts to revive their business to profitibility. This report provides analysis, evaluation, and recommendation for China Airlines. Strategic Difficulties • Outrageous crashes record (safety issue): there were 12 fatal crashes in its history that heavily discredited the image and revenue of CAL. All due to low standard of recruiting and training captain, pilots and crews, wrong policies and low standard of maintenance at the time (cost-cutting in maintenance), and desperate responses to save its image. • Constrain on China and Taiwan relationship: Taiwan’s airlines (flights and cargo) were required to land in Hong Kong or Macau before entering the mainland China, which wasted time and added extra unnecessary costs. Moreover, tourists travelling from China were not allowed to travel to Taiwan individually...
