Although I feel that all of these categories can apply in some way or another, the category I choose to develop my question on is the failure of the management- both operational and institutional. From reading Chapter 11 Foresight and Hindsight in the 9/11 Commission Report, we can see how the management missed numerous opportunities in upsetting the 9/11 plot. According to the Commission’s Report the reasoning behind this is because “Information was not shared, sometimes inadvertently or because of legal misunderstandings. Analysis was not pooled.
