Uncle Tommy dies from a fever that worsens over time. The reason why he dies from this sickness is that the civil war fighting that reaches Freetown leaves everyone scared to leave their houses in fear of being killed by rebels, including any nurse or doctor. No doctor is willing to go out of their way and risk being shot in order to aid Uncle Tommy. Without any professional attention to his sickness, it gradually gets worse and kills him.
