In the United States an average of 11% of the children between ages 4 and 17 are diagnosed with the medical disorder called ADHD (attention deficit hyperactivity disorder). ADHD is a common behavioral disorder that affects school aged children. This percentage has increased from 7.8% in 2003. With this disorder increasing 3% per year, some begin to wonder if it is the children that are needing to be “fixed” or is it the way we teach our children. Are there alternatives to helping children with their focus other the medication? Medicating children due to ADHD is not always the answer and often alters the child’s personality. There are alternate teaching methods available for students before having them medicated. Teachers can perform these teaching methods to help children focus better in school and not get bored. Parents can help by also delivering these methods at home.
Kids with ADHD may have trouble with focus, acting without thinking, hyperactivity and troubles knowing what is expected of them because they have a difficult time sitting and paying attention, so focusing on something is very difficult. The symptoms that they experience typically happen over a long period of time and do not just come in go like with children that just may be excited or anxious about something.
There are no test that can diagnose ADHD. In order to evaluate the child a Dr. will meet with the child in the office as well as getting evaluations from the schools and from home to be sure that there are not issues at home that are causing the behaviors. To be diagnosed with ADHD, a child must display behaviors from one of the three subtypes before age 12, these behaviors must be more severe than in other kids the same age, the behaviors must last for at least 6 months, and the behaviors must happen in and negatively affect at least two areas of a child's life (such as school, home, childcare settings, or friendships) (kidshealth.org)

One of the first treatment plans that doctors and schools recommend for ADHD is placing the child on stimulants. One common choice of medications is Ritalin. This drug can have long term effects such as psychological effects, Parkinson’s disease, brain development delays, drug abuse, and heart problems. Parents often are under educated about this drug and do not realize the effects of this drug and aid in the long term effect of this by placing them on this.
Stimulants have been used to treat ADHD for several decades. In studies, they have noted 70 to 80 percent of patients that use stimulants are effectively treated with no negative side effects. Stimulants help regulate the chemistry in the brain there for increasing the ability to control their behaviors. Adderall, Concerta, Focalin, Ritalin and Dexedrine are a few of the common medications that are given to treat moderate to severe ADHD. Some of the side effects of stimulant drugs are sleep problems, weight loss, decreased appetite, headaches, jitters, and emotional outburst. Other more serious effects could be cardiovascular problems, and severe depression and or anxiety. Medications can also alter the child’s personality. Children on these medications can become subdued making it seem as though they are focused more because they are too tired to cause issues in the class due to the lack of eating and sleep.
Not all children have the same ADHD. There are actually several types of ADHD and the wrong medication can harm more than help if not treated for the right type of ADHD. “The American Psychiatric Association has identified three subtypes of ADHD:
1. ADHD, Combined Type: Both inattention and hyperactivity-impulsivity symptoms
2. ADHD, Predominantly Inattentive Type: At least 6 out of 9 inattention symptoms, but not enough hyperactivity-impulsivity symptoms
3. ADHD, Predominantly Hyperactive-Impulsive Type: Hyperactivity-impulsivity, but not enough (at least 6 out of 9) inattention symptoms. (WebMD)
There are test that can be done to scan the brain to measure the blood flow in the brain that are related to short term memory, and attention. They then can describe six types of ADHD: * Type 1 -- Classic ADHD. Symptoms of ADHD, plus hyperactivity and impulsivity; responds well to stimulant medications * Type 2 -- Inattentive ADHD. Features of ADHD, but instead of hyperactivity, there is low energy; responds well to stimulant medications * Type 3 -- Overfocused ADHD. Symptoms of ADHD and negative thoughts and behaviors, such as opposition and arguing; tends to respond better to an antidepressant (such as Prozac) combined with a stimulant * Type 4 -- Temporal Lobe ADHD. The hallmark features of ADHD, plus irritability, aggressiveness, and memory and learning problems; responds better to antiseizure medications (like Neurontin) than to stimulants * Type 5 -- Limbic ADHD. Combines ADHD with depression and low energy and decreased motivation; responds better to stimulating antidepressants than to stimulants * Type 6 -- The Ring of Fire. Cross between ADHD and bipolar disorder; characterized by moodiness, aggressiveness, and anger. Anticonvulsants or newer antipsychotic medications tend to work better than stimulants. (Webmd)
The fact that not all ADHD respond to stimulant medications and the lack of research showing the long term effects of the medications, many are fearful of trying the medications and seek alternative natural medications or therapy to help treat ADHD. When the effects of stimulants are not effective or cause bad side effects the will then prescribe non stimulant medications like Strattera. This medication can improve concentration and impulse control. Then the question still remains though- what are we doing to our children long term but placing them on these medications? Can we offer other solutions without medicating first?
Today there are many different ways that we can help our children become more engaged in school and want to pay attention to what is being taught in school. Creating a routine and sticking with the routine is very important to children of all types, but kids with ADHD need the routine schedule so they can know what to expect throughout their day. All of the things that are incorporated in the classroom should be something all students can use so that we are not singling out the children with ADHD. Teachers could have planners or calendars for each child to be able to check off when they are complete so they know what is next. Minimizing the amount of teacher to student ratio in the class is also very import. Children with ADHD get easily distracted. Having many children around in the class doing multiple things can make it impossible for the child to focus on their school work. It also makes the child feel as though they need to compete with the other children for attention, so often that comes out in a negative way. The most effective way of helping ADHD in the classroom, is being more effective in your teaching. Visual activities, team work and interactive learning are ways to engage the children to learn while keeping them focused by having fun and moving. Children with ADHD have a difficult time when it comes to testing. The thought of having to focus is so overwhelming to them, that they lose focus on what they actually have to learn. With spelling- If you have the children say the words out loud then spell them on paper, it will encourage the child to hear and focus on the word, then be able to properly spell them.
When we incorporate learning plans in the classroom by offering lots of hands on learning, we could decrease the need for medication in children. Children have lots of energy. The more they can get up and move and be more involved with the activity the more engaged they will become. “Some accommodations can be made to help all children without singling out the child with ADHD. Having homework planners that make the child responsible for completing and following through on. Movement when the need for fidgeting, yoga balls to bounce on, visual reminders and walking to the office or designated area when there are needs to move.” (Department of Education)
Behavioral interventions are sometimes necessary to control the outburst and impulsive behaviors. Staying consistent on the awards and effects of negative and positive behaviors is important. “If a child can't focus, doesn't complete work, disrupts the classroom and is constantly in trouble, not only does the teacher have a problem, the child has a problem. A Behavior Intervention Plan is a document that describes just how the IEP team will help the child improve his or her behavior.” (Webster, 2015) Parents can also have a BIP plan to implement at home. Charts of success and awards when chores and good behavior happen can be a helpful visual aide for the child.
My son Justin was diagnosed with ADHD at the age of 5. His Dr. performed multiple test to determine his levels of focus when engaged in certain activities. He conducted a physical exam as well as a hearing and vision test to rule out other problems that may be happening. After his studies, he determined that Justin was border line ADHD. I had asked him if he felt that he needed to help with this at this point, or if it is something that can be helped with alternate teaching. He told me the choice was mine to make as he could not force me to put him on medication. He then requested a survey from all of his teachers asking questions about what they see in the class. How can this help? They were the ones that said he needed to be tested, they are not going to change their thoughts now, was the thoughts in my mind. Within 2 weeks my son was diagnosed ADHD with a learning plan (IEP) in school and starting medications to alter his behavior. I immediately began to see changes in my son. It and it was like a new child. I hardly recognized him. This was not for the better though. He became very aggressive and emotional. We were in the car one day on the way to a movie and he began crying so hard that I could not calm him down, about something that normally would not have affected him. The teachers all talked about how great he was and how much the medication was helping, and encouraged me to keep him on it. As a parent I thought, “I am not having my child live this way so that your job is easier!” They then suggested that I basically make my son a guinea pig and try him on different medications. They said that sometimes it takes up to 5 different medications before they determine which will work. Through this process children become very discouraged with themselves and don’t want to learn. They start thinking they are dumb and con not be helped.
If parents and teachers could work together to follow a plan that is customized to your child as well as be more educated of the child’s ADHD conditions and symptoms, they can help treat the ADHD without medications. Medications should be given as an alternative after all other things have be unsuccessful, instead of a first option. By providing clear instructions, making eye contact providing frequent prompts, and providing lots of physical exercise and movements to help with energy, we can encourage and promote the child’s strengths and provide education to the children without the harmful side effects of medication.
