•How important is it to network and collaborate with other providers?

Case management is the process of helping patients with lives that are not productive or satisfactory as a result of a number of problems among them drug and substance abuse. Case manager focus mainly on collaborating and developing by finding the client's strengths and abilities and makes a plan based on this. As this collaborate considers the wellbeing of the clients, so the participation of the client in the treatment is very important. The client must figure out their needs …show more content…
The case manager will give the client professional suggestions and make plans according to the client's condition which maybe not very consistent with the client's imagination and needs. But the client should be considerate since the accessibility and the efficiency. And the case manager should also have a regular contact with the client and be ready to make any changes to make sure the plan is always suitable and updated. Adult Protective Services where I worked, we have a referral list of all of the resources of Dallas County and ones that we have a working relationship with. Comparing with providers and the case management is relatively fixed because the client's hobbies and preference are stable. Based on this a long term plan is worked out and the plan will be seldom changed. But the plan is not very detail because it's just an optional orientation. Case managers are committed to keeping all the various resources and stakeholder groups informed and soliciting their input during the planning and implementation process.
•What is the role of the case manager in the referral process?

The case manager initial phase of the referral process is to determine the service need of our clients. The need is often determined as a result of a phone call received by a family member, or another service provider. Many service needs are determined at the intake when the client initially enrolls in case management services. This information is then provided to the permanent case manager so that they can begin the referral process. Needs are also discovered at the annual treatment planning meeting. At this time, the goals and objectives are reviewed and needs may change depending on client progress and preferences. The Child Protective Services department handles any case of abuse with an individual under the age of 18. After reading several CPS websites from different states, the general guideline is that anyone who notices abuse of a child has the obligation to report it to the authorities so they can investigate. The same goes for the APS, the general guideline is about the same when it comes to abuse of adults. With the adults, the criteria is that the individual must be at least 18 years of age and mentally or physically incapable of defending themselves against the abuser. These are just the basics of both departments and I feel that there are situations that are brushed under the rug, so to speak. The study also shown that two-thirds of the offenders were adult children or spouses. Additionally, elders are being abused in nursing homes, hospitals, or other institutions; in one study, 36 percent of nursing home staff stated that they had witnessed at least one physical abuse incident with an elderly patient (Nelson, 2002). Clearly, elder mistreatment or abuse of the elderly is a growing social occurrence. Child abuse reports have maintained a steady growth for the past ten years, with the total number of reports nationwide increasing 45% since 1987 (Nation Committee for the Prevention of Child Abuse (NCPCA) 2000 Annual Fifty State Survey). Neglect represents the most common type of reported and substantiated form of maltreatment. In 1996, 25 states provided the following breakdown for reported cases: 62% involved neglect, 25% physical abuse, 7% sexual abuse, 3% emotional maltreatment and 4% other. For substantiated cases, 31 states gave the following breakdowns: 60% neglect, 23% physical, 9% sexual, 4% emotional maltreatment and 5% other (NCPCA's 1996 Annual Fifty State Survey). In 1999, an estimated 1,401 child abuse and neglect related fatalities were confirmed by CPS agencies, nearly 4 every day. Since 1985, the rate of child abuse fatalities has increased by 39%. Based on these numbers, more than three children die each day as a result of child abuse or neglect (NCPCA's 1996 Annual Fifty State Survey). According to information from at least 18 states that were able to report the type of maltreatment which caused the child's death for at least one of the past three years.
