During my shift the building was quite. I completed three floor checks and found personal trash in both laundry rooms and all bathrooms, the lights between rooms 314 and 315, in the first shower stall of the first floor bathroom, and in the third shower stall of the third floor chapel side bathroom are out. As well as the second shower stall door in the third floor library side bathroom is off its hinges, and the salon sinks are clogged. Around 7:30 pm, Morgan Reese in room 213 came downstairs and reported that she slipped on a puddle that was on the stairs. I got her to do an incident report and I cleaned the water off of the stairs. After a couple of hours, I went to check on her. Her ankle was a little swollen and she had experienced some

...was conducting his security walk in D Pod. While Officer Kahn was in D Pod, I noticed inmates in C Pod grouping in the bathroom. One inmate had taken his shirt off and was looking towards the back of the bathroom.
