2.1 List the forms, registers and records that your organization uses to collect the information on WHS incidents, injuries and near misses. How could analysing these records help the PCBU or health and safety officer improve WHS performance in your organization?
• Hazard assessment checklist
• Inspection forms
• Incident reports
• Sickness absence records
• Medical records / employee history of exposure
• Worker’s compensation records
• Performance appraisal and training records

• The work environment is a health care setting and types of work include nursing duties, personal care assistant duties and cleaning staff duties.
• Methods used to explain WHS legislative and regulatory requirements include conducting WHS meetings routinely, using notice boards to update staff on regulation and sending information via staff email. Routine WHS training sessions are also conducted followed by a questionnaire to check individual staff competence.
• The types of issues discussed during WHS meetings include, changes to current regulations, incident reports which includes actual incidents and near misses and any changes to the work environment that may have an impact on WHS.
• Post meetings, staff will be sent an email briefing what was discussed at the meeting and what steps will be taken to address any concerns raised during the meeting. Staff will be continuously updated as necessary.
One of the issues raised was the shortage of staff which placed the rostered staff at risk. Follow up action includes the addition on 1 employee who will act as a floater and relieve certain duties of the rostered
