For the CCRC functional assessment assignment, I was assigned to a female resident at Clearwood House Retirement Community. My resident was a 95 years old female who was a widow and of Caucasian ethnicity. Her medical diagnosis was Type II Diabetes mellitus, Hyperlipidemia and Primary Hypertension. According to the resident, she classified her rating of health as being, "fair."
After conversing with my resident and performing the Spices Assessment, I chose to perform Activities of Daily Living and the Hendrich II Fall Risk Assessment. The reason I chose Activities of Daily Living is because when I first meet her she was having difficulty washing her hands. I chose to do this assessment because the resident showed evidence of being a fall risk. She was in a wheelchair and seemed to have trouble moving around. The patient was oriented by person, place, time and situation. I asked her whether she was able to use the restroom by herself and her response was, "I need some help using the restroom because I get dizzy from time to time, and sometimes I don’t make it on time" which gave her 2 points; 1 for altered elimination and 1 for dizziness. The "Rising from a Chair" test gave the resident 4 points because she was unable to get up by herself. Her total for the test was 6 points and classified her as a high risk for falls. An implication that arose due to the results of this test was to have a caregiver monitor resident for falls. She needs to have her room free from clutter on the floor to prevent obstacles in her way. She also needs to have a call light in close proximate to her so she is able to call for help if she needs to move somewhere or use the restroom. I assisted the resident in moving her phone due to the cords hanging in the front of the dresser to prevent her wheelchair from being tangled. She also needs to wear shoes with rubber soles or socks that have rubber bottoms on them to prevent her from slipping while she is in her wheelchair. Nurses will need to monitor the patient and make sure she is looked after while using the restroom and any activity that causes her to move around.
