On an annual basis the company loses about 3 million dollars due to rig’s averaging 3 lost time injuries per month and 9 non-lost time injuries. In May 1987 one of the rigs reached a milestone of working one full year without a lot-time injury. The president of the company was so impressed with the crew on the rig that we flew in personally to present the crew with a bronze plaque. On top of that the crew was awarded prizes averaging about $200 which were to be delivered in July 1987. But in June 1987, Joe Herbert a supervisor on the rig informed the human resource that he had an unreported accident in November 1986 and due to the accident he is going to need back surgery. Even though it was the company’s policy to record all accidents this accident was not recorded. To make things worse Joe had at ATV accident in May and due to him not reporting the accident right away. PART 2: POSSIBLE SOLUTIONS There are four key issues that need to be addressed; should the crew still be awarded the prizes, ,why wasn’t the incident reported, is there any way to prove that Joe’s back injury is caused from the ATV accident verse the work incident, and lastly are there any other safety programs should be in place. Committees are working on several areas where holes exist in the standards.2 5.4.1 Electrical Shock There is a danger of electrical shock with any motor vehicle should a mechanic or passenger come into contact with a live circuit under normal or fault conditions. With hybrid-electric buses, contact can come from electricity during battery charging (AC current) or discharging (DC current). If contact is made, the extent of injury will depend on the size, duration, frequency, and wave shape of the current. Conventional diesel buses make use of 12/24vDC and 220/240vAC. Effective RM requires that the process be integrated into all phases of mission or operational planning, preparation, execution, and recovery on a continuing basis. It is much more cost effective to plan up front during building construction and renovation to imbed safety, fire and environmental protection systems than to retro-fit after the fact. B. Make risk decisions at the appropriate level. As a decision-making tool, RM is only effective when the information is concentrated on the appropriate supervisory level for decision. The higher the risk, the higher the management level of who should make the decision to accept a risk or not. Often this will require the decision to apply resources, whether manpower, dollars or both, to mitigate risks to an acceptable level so the management decision-making level must be where the purse strings are controlled. C. Accept no unnecessary risk. Accept no level of risk unless the potential gain or benefit outweighs the potential loss. RM is a decision-making tool to assist the supervisor or individual in identifying, assessing, and controlling risks in order to make informed decisions that balance risk costs (potential losses) against mission benefits (potential gains).
