Scenario:
S.R. is a 69-year-old man who presents to the clinic because his “wife complains that his snoring is difficult to live with.”

1. As the clinic nurse, what routine information would you want to obtain from S.R.? (2 points)

I would want to obtain a smoking history (if any). I would want to know about their sleep: the hours of sleep obtained per night, if they are waking frequently through the night, their posture during sleep. Information I would want to obtain would also include if they wake up feeling tired, if they awake frequently during the night, if they fall asleep during the day, if they awake with a headache, and if they are irritable or angry in the morning. I would want to assess vital signs (to include oxygen saturation level), history of recent illness/respiratory infection, any changes in weight, trouble breathing through the nose, stressors, and difficulty concentrating. I would also ask the wife how loudly he snores and if she notices a pause in his breathing.

CASE STUDY PROGRESS
After interviewing S.R., you note the following:

* S.R. is under considerable stress; * he owns his own business; * the stress of overseeing his employees, meeting deadlines, and carrying out negotiations has led to poor sleep habits; * he sleeps 3 to 4 hours per night; * he keeps himself going by drinking 2 quarts of coffee and smoking three to four packs of cigarettes per day; * he has gained 50 pounds over the past year, leading

Case Study #7

...home (especially birds and cats) Pertinent nasal problems: deviated septum, chronic sinusitis Allergies: particularly airborne particles such as animal dander, dust mites, cockroach droppings Medications he is currently taking: opioids, modafinil (Provigil), amphetamines (prescribed and illegal), OTC drugs, and herbals 2. The two main types of sleep apnea are Obstructive Sleep Apnea (OSA) and Central Sleep Apnea (CSA). OSA is the more common type and is usually a result of obesity. Obstructive sleep apnea occurs when the muscles in the back of your throat relax. These muscles support the soft palate, the triangular piece of tissue hanging from the soft palate (uvula), the tonsils, the side walls of the throat and the tongue. When the muscles relax, your airway narrows or closes as you breathe in, and you can't get an adequate breath in. This may lower the level of oxygen in your blood. Your brain senses this inability to breathe and briefly rouses you from sleep so that you can reopen your airway. This awakening is usually so brief that you don't remember it. Central sleep apnea or CSA is a less common form of sleep apnea that occurs when your brain fails to transmit signals to your breathing muscles. This means you make no effort to breathe for a short period of time. You may awaken with shortness of breath or have a difficult time getting to...

Sleep Management

...Nursing Management of Sleep Disorders March 2013 INTRODUCTION Sleep is the state of natural rest observed throughout the animal kingdom, in all mammals and birds, and in many reptiles, amphibians, and fish. In humans, other mammals, and many other animals that have been studied - such as fish, birds, ants, and fruit-flies - regular sleep is necessary for survival. The capability for arousal from sleep is a protective mechanism and also necessary for health and survival. DEFINITION Sleep can e defined as a normal state of altered consciousness during which the body rests; it is characterized by decreased responsiveness to the environment, and a person can be aroused from it by external stimuli. INCIDENCE & CHARACTERISTICS: Sleep is generally characterized by a reduction in voluntary body movement, temporary blindness, decreased reaction to external stimuli, loss of consciousness, a reduction in audio receptivity, an increased rate of anabolism (the synthesis of cell structures), and a decreased rate of catabolism (the breakdown of cell structures. Almost a third of the general population has some problems with sleep during any given year. More than half of the 9000 participants in a study of sleep in elderly persons (65 years or older) reported the following as sleep pattern disturbance that they experience most of the time: • Trouble falling asleep • Frequent awakening • Waking too early • Needing to nap • Not feeling rested These disturbances may be secondary...

Obesity

...CHAPTER 1 INTRODUCTION Definition Obesity is a medical condition in which excess body fat has accumulated to the extent that it may have an adverse effect on health, leading to reduced life expectancy and/or increased health problems. People are considered obese when their body mass index (BMI), a measurement obtained by dividing a person's weight in kilograms by the square of the person's height in metres, exceeds 30 kg/m2. Obesity increases the likelihood of various diseases, particularly heart disease, type 2 diabetes, obstructive sleep apnea, certain types of cancer, and osteoarthritis. Obesity is most commonly caused by a combination of excessive food energy intake, lack of physical activity, and genetic susceptibility, although a few cases are caused primarily by genes, endocrine disorders, medications or psychiatric illness. Evidence to support the view that some obese people eat little yet gain weight due to a slow metabolism is limited; on average obese people have a greater energy expenditure than their thin counterparts due to the energy required to maintain an increased body mass. Dieting and physical exercise are the mainstays of treatment for obesity. Diet quality can be improved by reducing the consumption of energy-dense foods such as those high in fat and sugars, and by increasing the intake of dietary fiber. Anti-obesity drugs may be taken to reduce appetite or inhibit fat absorption together with a suitable diet. If diet, exercise and medication are...

INTRODUCTION University life has presents many new and stressful challenges, such as reduced parental supervision which may increases freedom, self-responsibility, disorganized lifestyle, variable schedules, repeated deadlines, dormitory living, new social opportunity and academic obligations (Anbar and Slothower , 2006) . All of these caused students voluntary to alter their sleeping habits in which they tends to reduce their sleeping time from as recommended sleeping time 8 hours to in between 3 hours to 6 hours of sleeping time per day. This can lead to sleep disorder in which nowadays, insomnia and sleepiness are the most common complaints in general population (Watson, Goldberg, Arguelles, and Buchwald, 2006). Notably that, sleep quality refers to a composite of sleep quantity, length of time to fall asleep, number of awakenings at night, length of time to fall back asleep after awakening, feeling of fatigue or restfulness upon awakening in the morning and general satisfaction with sleep. According to Doghramji (2006), insomnia can be defined as complaint of disturbed sleep, manifested as difficulties in sleep initiation or sleep maintenance, and also refers as early awakenings. According to Kamel and Gammack (2006), there are several causes of insomnia which are including primary specific sleep disorders, physical illness, behavioral, environmental and medication. General Psychology

...Chapter 1 The Dimensions of Psychology Summary: Psychology is an academic and applied discipline that involves the scientific study of mental functions and behaviors. Psychology has the immediate goal of understanding individuals and groups by both establishing general principles and researching specific cases, and by many accounts it ultimately aims to benefit society. In this field, a professional practitioner or researcher is called a psychologist, and can be classified as a social, behavioral, or cognitive scientist. Psychologists attempt to understand the role of mental functions in individual and social behavior, while also exploring the physiological and neurobiological processes that underlie certain cognitive functions and behaviors. Structuralism was the first school of psychology and focused on breaking down mental processes into the most basic components. Researchers tried to understand the basic elements of consciousness using a method known as introspection. Wilhelm Wundt, founder of the first psychology lab, was an advocate of this position and is often considered the founder of structuralism, despite the fact that it was his student, Edward B. Titchener who first coined the term to describe this school of thought. Functionalism formed as a reaction to the structuralism and was heavily influenced by the work of William James and the evolutionary theory of Charles Darwin. Functionalists sought to explain the mental processes in a more systematic and accurate...

Effects of Insomnia

...inability to sleep adequately a few times a weak and doesn’t mean the inability to sleep at all. Insomnia can be chronic or on going.Certain causes of insomnia can be categorized into three major categories which include lifestyle, health and environment. Experiments were carried out to see how insomnia affects quality of life and the conclusion reached was that people suffering from insomnia report greater quality of life impairment in contrast to people without insomnia. Sleep disturbance can exacerbate the condition, disrupt treatment, cause further complications and add to the social disability associated with chronic illness. Insomnia can result in mental illness and can have a huge impact when it comes to efficiency of a subject. Personally traits can be a major differentiating point when it comes to comparison between subjects with and without insomnia however personality differences between insomniacs and controls might rather be state-dependent instead of trait-dependent. Treatment of insomnia can be by two major ways namely pharmacotherapy and nonpharmacologic interventions. Herbal treatment as per 82% of Canadians are also very effective when it comes to treating insomnia. Rest and little exertion can also play a vital role when it comes to curing insomnia. Effects Of Insomnia On Quality of Life Insomnia is derived from the Latin word which means “no sleep”.
