Family: Caregiver and youth struggles with communication due to youth’s lack of linguistic skills. Ms. Warren reports that she wants youth to learn basic living skills. Ms. Warren is considering placing youth in an Autism Group Home. CM encouraged Ms. Warren to continue to advocate for the youth and CM suggested the family to look into family counseling.

Behavior: Ms. Warren reported that youth can be impulsive at times and youth can be easily redirected. Ms. Warren informed CM that youth does not know how to make choices by herself without guidance.

Medical: Youth has scoliosis and needs to wear a back brace 24/7.

A longiudinal study of ex-institutional children was done by Hodges and Tizard in 1989, to investigate the effects of privation by following the same children over a long period of time to collect reliable information linking early experiences to later outcomes for the same individual. The participants were 65 children who had been placed in an instituation when they were less than 4 months old. There was an explicit policy in the institution against caregivers forming attachments with the children. This would suggest the children experienced early privation. By the age of 4, 24 of the institutionalized children had been adopted, 15 had returned to their natural homes and the rest remained in the institution. Assessment at the age of 8 and 16 years old involved interviewing those children who were adopted and those who had returned to their original homes. Their parents, their teachers and their peers were also interviewed. Data was also collected from a control group of ‘nomal’ peers. Reviewing 24,423 citations yielded 21 relevant qualitative studies, four systematic reviews, and eight additional intervention studies. The qualitative literature described the domains of accessibility and coordination; competence, including symptom management; communication and education; emotional support and personalization of care; and support of patients’ decision-making. For collaboration and consultation interventions, eight of 13 studies showed a significant effect on satisfaction. A metaanalysis found that palliative care and hospice teams improved satisfaction, although most studies did not include satisfaction as an outcome. For other types of interventions, only two of six showed a significant effect.
