CCIB Intake received the Corrective Action Plan from San Gabriel/Pomona Regional Center. Quality Assurance Specialist Jaime Anabalon met with Margie Canlobo on 12/8/16 to discuss and develop a corrective action plan regarding a delay in seeking medical treatment. Also present at the meeting was Rebecca Jordan, Residential Services II Manager, Gloria Rodriguez, Quality Assurance Liaison, and Dara Mikesell, Quality Assurance Manager. On 11/24/16 family member of resident (LG) picked her up for dinner and returned her to the facility later that same evening. The family member who dropped the resident off alerted staff to bruising they had noticed on the inside of her left arm. The administrator reported the resident had been prompted to go to
