Challenger Disaster On the morning launch of the challenger on January 28th, 1986, at approximately 9:41am an unexpected event happened. The shuttle, within 75 seconds of taking off, exploded in the air. Citizens and NASA staff were shocked and confused as to what they had just seen, but there was a small group knew this type of problem could occur. They had little evidence but still chose to launch that day, now worrying about the events that will transpire thereon. Many questions as to why the shuttle had exploded arose such as, “What caused it to malfunction?” “Was there any known facts before the incident?” And if so, “why was there no action taken?” Unfortunately, some of these questions were addressed and brought to a few personnel in top management positions such as managers, vice presidents, supervisors, and engineers. One of the main issues was the failure to communicate the problem of the O-rings in a timely manner. The fact that they waited until the night before the launch to discuss the problems that the seals were causing, was an obvious point leading us to believe that the organization of communication within the chain of command was faulty. Another issue was that Larry Mulloy saw the data that Roger Boisjoly and Arnold Thompson provided, lacked details to support their theories. In all retrospect, they were the ones who had done the research and work that lead them to their conclusions. It was obvious that they knew more about the outcomes of launching the shuttle at a temperature lower than 53° F, would have a serious impact on the O-rings not sealing/performing well, and causing a greater problem (the loss of human life). Thiokol engineers also knew about the impending danger that the O-rings would cause. They stated their position on the matter, in hopes that NASA would act fast on the matter,
“It is my honest and very real fear that if we do not take immediate action to dedicate a team to solve the problem with the field joint having the number one priority, then we stand in jeopardy of losing a flight along with all the launch pad facilities.” (Case study 1)
NASA’s conclusion to this was that it wasn’t a major issue of shutting down the whole fleet, they simply asked them to fix it as they went along. It was definitely clear that NASA was being pressured to launch, because they “wanted challenger in orbit while President Reagan gave the State of the Union address”. (Case study 1) By being politically pressured to launch challenger, all NASA wanted was to have all issues fixed along the way by the different departments, and just have everything ready, problem free ready for launch. Their values were being pushed to the side, their objective was becoming harder to achieve, and the tools that would help them in situations like this were not being used properly. If the teleconference meeting was being held today, and I was in Roger’s shoes, one approach that I would take to garnish support for my position on the matter would be that I am a member of the seal task force and I know more of what the O-rings consist of, and how and why they would not work well in certain conditions and temperatures. Working closely with the seals is all I do, so yes, I am the one who has done most of the research and work, despite the data that is being presented. Discussion about the issues at hand about the O-rings and how they work will help establish my point of view on the matter. In order to manage the opinions and thoughts of the management team, it would be to have them listen well to the facts, and that they look at it from the engineer’s point of view. Trust the instinct of the expert who knows more about the issue at hand, and really try to understand and support. If only they weren’t so stubborn, they could have prevented the launch from happening, or moving to a later time of day, thus saving the lives of the 7 astronauts and addressing the problems of the seals. If only they supported their teams, like Shackleton supported and listened to his crew mates in times of disaster or weather changes and stress. Being a leader just isn’t leading, but acting responsibly, listening and making a good decision, without allowing your values to be thrown out the window because of political pressures. I believe this event could have been prevented, if only the management higher ups would have listened and tried to understand what the problem at hand was, and would have just supported their department experts who do the research and work.
