Circumstance: Ms. Smalls (MHP) made monthly contact with Deonte and family.
Action: MHP conducted monthly visit with Deonte and family. Alex and Ryan (MHS) reported no behavioral problems in the home or community. Deonte provide MHP a walk-through of the home to share his Christmas tree in his bed room and living room area. MHP and MHS review paperwork. MHP and MHS discuss Deonte having sibling visitsion. MHS and Deonte share Christmas pictures and plans for the holiday. MHP and Deonte school and following the rules.
