On December 24th 2017 you were actively engaged in the task of performing routine weapon maintenance of your department issued Glock Model 22, .40 caliber handgun (serial #LYK452). Subsequent to completing the maintenance, you reassembled the weapon placed a loaded ammunition magazine into the magazine well. You then cycled the action of the weapon loading one round of ammunition from the magazine, into the chamber making the weapon in a condition that it is ready to fire the chambered round.

At that time you were requested by a trainee to demonstrate the disassemble procedure for the Glock handgun. You then removed the loaded magazine from the weapon, and began the disassembly process. You then failed to properly unload

...At about 9:30 a.m. on December 14th in 2012 a 911 call was made that there was a possible shooter in the Elementary school. Just under three minutes later police responded to find that a tragedy had in fact already occurred. These deaths were at the hands of a twenty-year-old boy named Adam Lanza. On the day of the shooting prior to driving to Sandy Hook, Lanza also took the life of his mother, Nancy Lanza. After killing her he drove to Sandy Hook and massacred twenty-six people with a Bushmaster AR-15 rifle. Lanza wore all black clothes and a bullet proof vest while carrying the AR as well as two handguns one Glock and one Sig (CNN, n.d.). At his arrival the doors to the school were locked, as they locked at 9:30 a.m. every day. He proceeded to use his AR to shoot through the door and gain entry to the school. When he entered the building he immediately shot the school psychologist, Mary Sherlach, killing her. He also shot the Vice principle who was with her, thankfully she was only wounded and survived.
