The improvement in clinical documentation is one area that will affect the number of charts on hold at facilities for documentation issues like physician queries. This is critical especially in procedure coding where the interest is not the amount of documentation that the provider will or will not accumulate, but the specificity of that documentation (Arends-Marquez, Knight, & Thomas-Flowers, 2014).
