Introduction
Rationale
There are two primary reasons why healthcare workers (HCWs) have been targeted for influenza (flu) vaccination. The first reason is to protect HCWs from disease and to prevent work absenteeism, particularly during flu outbreaks or pandemics. The second motive is to protect others such as co-workers and family members, especially vulnerable patients in their care since HCWs can be a significant source of nosocomial spread of influenza. When a healthcare professional receives a vaccination, it sets an example for colleagues and others in the community and increases the likelihood HCWs will advocate for vaccination.

Although the CDC has made this recommendation to staff in healthcare settings regarding vaccination policies, they have also allowed institutions to determine what protective measures to enact (Centers for Disease Control and Prevention [CDC], 2015). A variety of strategies have been proposed and implemented to improve influenza vaccination rates among HCWs. These measures have included offering free vaccination at convenient work sites, providing HCWs with enhanced influenza education, providing modeling and support for influenza vaccination by leaders within healthcare institutions and requiring signed statements from those HCWs who decline vaccination (CDC, 2015). Historically, the response of HCWs to voluntary vaccination programs has failed to increase vaccination rates to national target levels (Association for Professionals in Infection Control [APIC], 2011). A variety of programs have been directed at improving vaccination uptake but national rates of influenza vaccination coverage for HCWs have remained low at approximately 63.5% during the 2010-2011 influenza season due to barriers that included concerns about adverse reactions, low perceived susceptibility to influenza infection and the inconvenience of obtaining a vaccination (Naleway, Henkle, Ball, Bozeman, Gaglani, Kennedy, …show more content…
& Pergam, 2015). Data validated the success of this model with reports of vaccination coverage among HCWs at Fred Hutchison increasing from 88% in 2010 to 96% in 2012 (Podczervinski et al., 2015). This study also confirmed that a high level of compliance for vaccination programs could be achieved without resorting to mandatory vaccination policies. The “carrot” approach of changing minds with monetary interventions and the “stick” with the burdensome time required to complete the declination process is an evidence-based strategy that will benefit other healthcare facilities (Podczervinski et al., 2015).
Intervention
The Health Belief Model: A Guide for Influenza Prevention among HCWs The Health Belief Model (HBM) offered a method to explore why HCWs may accept or decline flu vaccination. In response to a specific threat, such as contracting influenza, the HBM is an effective tool for examining an individual’s perceptions related to the following:
• Demographic Variables: HCWs (males and females) at Harbor View Medical Center
• Perceived Susceptibility: HCWs have a low perceived susceptibility to flu and may feel it is not a serious illness, flu vaccine is not essential for HCWs,
