The Insured claimed that she was hired as a Front Office Receptionist where she works at a medical office facility called the ChapCare Medical Services company. She said the company that she works with contracts with several qualified health plans to provide primary health care services for an assorted health insurances companies which will accept whatever health insurance coverage plans that the patient has currently. She described her job duties chronologically by starting off by saying that her primary responsibility is to make many phone calls with several health insurance companies for the patient to check for eligibility and the care needed for the patient. The amount of phone calls it takes to fulfill the duties of her position consists …show more content…
She is also required to work one Saturday of the month where she would work from 8:30 AM to 1:00 PM. Her full-time work schedule requires her to work 40+ hours a week where she received two fifteen-minute breaks and one thirty-minute lunch break. She earns an hourly wage of $21 per hour and receives health insurance which she has elected to have Kaiser Permanente as her primary insurance coverage for her and her family.

Her current position requires her to have a Medical Administrative Assistant Certification, as well as the year of experience with customer service skills in communicating with patients. The job also requires for her and to anyone else who would fill the same position is required to type approximately 35 words per minute while being knowledgeable of the EPIC software.

After her last day worked on April 11, 2017, the Insured maintained that she has not attempted to work in the capacities after she had filed her disability with UNUM. Even though she has not received any checks or monetary funds from UNUM, she claimed that she has been tempted to work in any other similar capacities with any other employer. Furthermore, she stated employed with ChapCare she did not have any concurrent work. He maintained, that she’s never attempted any similar type of work which she performed with her current employer to supplement her income or gain full-time employment with a similar
