Metformin replaced by Injectable insulin Lantus. THER -antidiabetic, PHAR - biguanides Decreases ephatic glucose production, and decrease intestinal glucose absorption
Increases sensitivity to insulin (Vallerand, Sanoski, & Deglin, 2013).
O.M diagnosed w/ type 2 diabetes Control diabetes. Pt. who has been well controlled on metformin who develop illness or laboratory abnormalities should be assessed for ketoacidosis or lactic acidosis. Assess serum electrolytes, ketones, glucose, and, if indicated blood pH, lactate, pyruvate, and metformin levels. If either form acidosis is present, discontinue metformin …show more content…
THER –antianginals, antihypertensive PHAR – beta blockers Blocks stimulation of beta1 (myocardial)-adrenergic receptors. Does not usually affect beta2 (pulmonary, vascular, uterine)-adrenergic receptor sites.
THER Effects: decreased BP and heart rate. Decreased frequency of the attacks of angina pectoris. Decreased rate of cardiovascular mortality and hospitalization in patients with heart failure. (Vallerand, Sanoski, & Deglin, 2013).
Control Hypertension. Monitor BP, ECG, and pulse frequently dose adjustment and periodically during therapy.
Monitor vital signs and ECG every 5-15 min during and for several hrs after parenteral administration. If heart rate <10/min, assess level of sedation. Physical stimulation may be sufficient to prevent significant hypoventilation. Dose may need to be decreased by 25-50%. Initial drowsiness will diminish with continue use (Vallerand, Sanoski, & Deglin, 2013).

Pain control for the diabetic ulcer. Assess overall health status and alcohol usage before administering acetaminophen. Pt. who are malnourished or chronically abuse alcohol are at higher risk of developing hepatotoxicity w/ chronic use of usual doses of this drug.
Increase serum bilirubin, LDH, AST, ALT, and prothrombin time may indicate hepatotoxicity
If overdose occurs, acetylc ysteine is the antidote (Vallerand, Sanoski, & Deglin, 2013).
Famotidine
(PEPCID 20 MG TAB) 20 MG THER -antiulcer agents
PHAR – Histamine H2 antagonists. Inhibits the action of histamine at the H2 – receptor site located primarily in gastric parietal cells, resulting in inhibition of gastric acid secretion.
THER Effects: Healing and prevention of ulcers. Decreased symptoms of gastroesophageal reflux. Decreased secretion of gastric acid (Vallerand, Sanoski, & Deglin, 2013).
Preventing gastric ulcers secondary to stress and medications. Assess for epigastric or abdominal pain and frank or occult blood in the stool, emesis, or gastric aspirate.
Monitor CBC with differential periodically during therapy (Vallerand, Sanoski, & Deglin,
