Introduction:

Healthcare organizations accredited by the Joint Commission are required to conduct a root cause analysis (RCA) in response to any sentinel event such as the one described below. Once the cause is identified and a plan of action established, it is useful to conduct a failure mode and effects analysis (FMEA) to reduce the likelihood that a process would fail. As a member of the healthcare team in the hospital described in this scenario, you have been selected as a member of the team investigating the incident.

Scenario:

It is 3:30 p.m. on a Thursday and Mr. B, a 67-year-old patient, arrives at the six-room emergency department (ED) of a sixty-bed rural hospital. He has been brought to the hospital by his son and neighbor. At this time, Mr. B is moaning and complaining of severe pain to his (L) leg and hip area. He states he lost his balance and fell after tripping over his dog.

Mr. B was admitted to the triage room where his vital signs were B/P 120/80, HR-88 (regular), T-98.6, R-32, and his weight was recorded at 175 pounds. Mr. B. states that he has no known allergies and no previous falls. He states, “My hip area and leg hurt really bad. I have never had anything like this before.” Patient rates pain at ten out of ten on the numerical verbal pain scale. He appears to be in moderate distress. His (L) leg appears shortened with swelling (edema in the calf), ecchymosis, and limited range of motion (ROM). Mr. B’s leg is stabilized and then he is further evaluated and discharged from triage to the emergency department (ED) patient room. He is admitted by Nurse J. The admitting nurse finds that Mr. B has a history of impaired glucose tolerance and prostate cancer. At Mr. B’s last visit with his primary care physician, laboratory data revealed elevated cholesterol and lipids. Mr. B’s current medications are atorvastatin and oxycodone for chronic

Professional Development of Nursing Professionals

...to the recommendations on the IOM report regarding transforming education, transforming practice, and transforming leadership. (Institute of Medicine, 2011) Impact on Nursing Education The IOM report made a key point in which “nurses should achieve higher levels of education and training through an improved education system that promotes seamless academic progression” (Institute of Medicine, 2011, p. 164). As the year’s progress it is vital that nursing education be transformed to stay current with today’s evolving health care. The IOM reports that even though the nursing profession must be transformed to work in a variety of complex settings however, they recognized that the primary goal of nursing education should still be that nurses prepared to meet diverse patients’ needs; function as leaders; and advance science that benefits patients and the capacity of health professionals to deliver safe, quality patient care. (Instiute of Medicine, 2011) The IOM report emphasized on the importance of having nurses with a higher education therefore, the committee recommended that bridge programs be made for the Registered Nurse (RN) with an associate degree in nursing (ADN) or diploma to transition into a program to complete a baccalaureate degree in nursing (BSN). Other highly encouraged educational pathways are ADN to master’s of science in Nursing (MSN), BSN to MSN,...

The Magnet Certification

...performing the health care needed solutions to solve global issues, such as nursing shortage, and the way the health system is developing. Based on research utilization proposals and highly trained staff, from management training, as well to support, and to nurse executives patient safety and health care quality is applied based on performance improvement projects. The components of a Model Magnet are exceptional professional practice, transformational leadership, empowerment, based on knowledge and innovation (American Nurses Credentialing Center, 2012). Magnet is something that demonstrates achievement. Magnet standards are also a set of standards to move toward (Chaney, 2002). The Forces of Magnetism According to, the American Nurses Credentialing Center (2012), in 1983 a research study was performed that identified 14 characteristics that differentiated the hospitals best able to recruit and retain nurses. These characteristics became the “Forces of Magnetism” that form the conceptual framework for the Magnet Recognition Program. They are a designation that indicates excellence in nursing practice. According to the ANCC, the forces are described as high-quality nursing leadership, an organizational structure that places nurses in executive positions, a management style that values and encourages employee feedback, flexible, forward-thinking personnel and staffing policies, continuous quality-improvement initiatives, adequate resources, in the form of nursing experts...

...(NP) requires coursework and clinical rotations beyond that required of the RN. Curriculum includes, but is not limited to, courses in epidemiology; nursing theory; health promotion; pathophysiology; physical assessment; pharmacology; differential diagnosis and laboratory/radiography diagnostics; statistics and research methods; health policy; role development and leadership; acute and basic chronic disease management (e.g., adults, children, women's health, geriatrics, etc.); and, clinical rotations, which varies depending on the program. MSN programs also require a clinical research project. There are a variety of paths to becoming a licensed nurse practitioner in the United States. Typically, the process begins with obtaining a Bachelor of Science in Nursing (BSN, usually 4 years), followed by a Master’s of Science in Nursing (MSN, usually 50 credits). The flexibility of the MSN degree allows students to work full-time as a bedside nurse while completing the coursework - a task that would be impossible in the rigorous medical training to become a physician or physician assistant. DNP programs are similar to the MSN programs in content and education. Doctor of Nursing Practice (DNP) programs require basic coursework in biostatistics; research methods; quality improvement and outcome measures;...

Nurse Leader Interview Paper

...Nurse Leader Interview Paper Uchenna Enewally University of Phoenix NUR 587 February 25, 2013 Breta Lieke, RN, MSN, NE-BC Nurse Leader Interview Getting a chance to interview a true nurse leader is not easy as many attempts to get an appointment fail. Nurse leaders have very busy schedules because of their organizational roles and finding time to grant a quality interview about their leadership style is slightly out-of-the-box for their daily routines. Interviewing a nurse leader is even harder when the interviewer is working from a remote area in Africa and has to do the interview by phone. The interview must take place so when finally MK, the Patient Advocate/Patient care services of St. Francis Medical Center consented and gave the interview, it became possible to report the personal in-sight of a good nurse leader. This interview report is a summary of MK’s perspectives on her leadership style, changes made because of evolving leadership role, challenges faced as a leader in today’s health care system and the effects of formal and informal power in the organization. Summary below are her answers to the questions asked by the nursing graduate student presenting this report. Leadership Style MK started at the organization in 2004 as the patient advocate manager to improve the customer satisfaction rating and advance the image of the hospital in the Los...

Personal Expreince with Strategic Planning

...Personal Experience with Strategic Planning Denise M. Chaney Creighton University Strategic Planning and Management ILD 803 Dr. Fink May 4, 2013 Personal Experience with Strategic Planning Strategic planning is the essence of the work of an organization. According to Bryson (2011), strategic planning identifies where the organization wants to be at some point in the future; without a strategic framework senior management will not know where they are going. Thompson, Peteraf, Gamble, & Strickland III, (2012), state “crafting and executing strategy” should be a top priority for those holding senior management positions. It seems that such a powerful entity such as strategic planning, should be well known throughout the organization. However, the opposite seems to be true. I’ve been in the field of nursing for more than 38 years and had never heard the term “strategic planning”. Is there a lack of knowledge pertaining to strategic planning? If so, whose fault is it, the organization or mine? According to Wells (1998), the best way to “reduce someone else’s capacity or humanity, severely restrict or take away their right to think” (kindle Fire, Location 142). Therefore, restricting access to “strategic planning”, employee rights have been taken away. Wells (1998), also points out “that the only limit” to an organization’s possibilities is “the mind of it’s people and what they are able to conceive (Kindle Fire, Location 46). In part, this reasoning could account for my lack...

Changing for the Better

...Changing for the Better Cathy Winchester Grand Canyon University Nursing History, Theories, and Conceptual Model NRS-430V Lorilee McAfee October 31, 2015 Changing for the Better The 2010 Affordable Care Act has changed the way health care needs will be delivered to the American people. It requires transforming the health care system to meet new challenges such as, quality, affordable and safe care that will require the roles of health care professionals, including nurses, roles to be reevaluated ("Health Care Reform," 2015). With that said, the Institution of Medicine (IOM) released a report in 2010 that states, “nurses’ roles, responsibilities, and education should change significantly to meet the increased demand for care that will be created by health care reform and to advance improvements in America’s increasingly complex health system” ("Health Care Reform," 2015, p. 1). The 2010 IOM report has great impact in the nursing field that will promote positive changes. In this paper I will discuss the impact of the IOM report on nursing education, nurse’s role as a leader, and nursing practice, particularly in primary care. Lastly, an explanation of how one can change their current practice to meet the goals of the IOM report will be discussed. The IOM report calls for nurses to achieve higher levels of education. One of the impacts to nursing education is the goal of increasing the percentage of nurses with a bachelor’s of science in nursing (BSN) to 80 percent by...

Value of Mba

...Graduate education is expensive and time intensive. In spite of these factors, obtaining a Master’s degree in business (MBA) is still worth all the time and expense because of the value it serves. Having an MBA will help to make me more marketable in the health care industry and more likely to be considered for a management or administrative position. In the following pages I will discuss the reasons that, with the career goal of becoming a nurse executive, the value of an MBA degree outweighs the struggles that I may encounter while in the graduate program. In addition to acquiring a Master’s degree in nursing, to be a successful nurse executive one must also have some knowledge of how a business runs. The American Association of Nurse Executives (AAONE) lists standard competencies for the nurse executive. These competencies are outlined in a 2008 article in Nursing Economics and include skill sets in areas of financial management, human resource management, strategic management, and information management and technology. A nurse executive must be able to represent the nursing population while understanding the mechanics of the business. A nurse executive who can understand financial implications of clinical decisions and fiscal matters is a huge asset to the business aspect of the organization (Thomas et al., 2008). All of the competencies that the AAONE considers standards for nurse executives can be learned through an MBA program therefore making having an MBA an advantage...

Effective Communication

...Effective communication through leadership is an essential tool in creating vision in the workplace and involves organization, collaboration, breaking down barriers, and the ability to read non-verbal cues. Without effective communication, inadvertent misunderstandings can occur, leaving the workplace vulnerable to collapse. The leadership role in any organization is the key to effective communication throughout the company. Leaders who effectively communicate the vision and mission of their company are generally successful because they exercise collaboration with the employees as to what works and what does not work. We learn to communicate at a very young age. However, to effectively communicate complicated ideas, skills beyond simple conversation are required (Stennes p. 1). One key component that is necessary when communicating vision with others is organization. Organization of what you want to communicate is necessary because it provides your listener with tools needed to understand the concepts that are provided. The organization of the conversation allows the listener to decipher the important details rather than dealing with the nonsensical information that may be included. Another key component when effectively communicating is collaboration. Collaboration is crucial in communication because it serves as a two-way process involving an exchange of ideas (Stennes p. 2). If a leader tries to take control of the conversation, the listener will most...
