Treatment
The treatment of Myasthenia Gravis is dependent upon whether it is seen as an autoimmune disease or a congenital neuromuscular disease within the patient. As an autoimmune disease, MG is treated through the usage of immunosuppressive drugs, medication with the intended goal of reducing the efficacy of the immune system. Some of these drugs include prednisone, azathioprine, cyclosporin, and mycophenolate, in order from most commonly used to least prevalent. Other less commonly used forms of treatment for autoimmune MG includes plasmapheresis, intravenous immunoglobulin therapy, and thymectomy. In general, MG results in antibodies focused against the nicotinic acetylcholine receptor, the cholinergic neuron receptor protein which signals …show more content…
It mimics the corticosteroids produced by the cortex of the adrenal glands, and ultimately reduces acetylcholine receptor antibody levels. As seen through a University of Virginia study, when treating 116 patients with prednisone, 52% improved significantly, 15% improved moderately, and 28% went through pharmacologic remission. Azathioprine is the second most prevalent immunosuppressive drug, and is generally used as an alternative to prednisone, (when the patient is intolerant or unresponsive to prednisone), or with prednisone (in order to decrease the dosage of prednisone used). The reason for using azathioprine as a secondary form of treatment is due to the fact that its beneficial effects take months to completely help the patient, whereas prednisone acts much more rapidly. Azathioprine is an antimetabolite, which functions by blocking cell proliferation. This is done by the inhibition of T lymphocytes, resulting in a decrease in acetylcholine receptor antibody levels.17 Cyclosporine was created with the intention to prevent rejection within organ transplantation patients. However, it is very effective for MG as it inhibits helper T lymphocytes, while simultaneously facilitating suppressor T lymphocytes, and also blocking both the production and secretion of interleukin-2. These drugs function by inhibiting acetylcholinesterase (an enzyme that breaks down acetylcholine) in order to increase both the duration and level of acetylcholine action, thus increasing neuromuscular transmission and assisting in muscular contraction. Pyridostigmine competes with acetylcholine for attachment to acetylcholinesterase in order to slow down acetylcholine hydrolysis. On the other hand, neostigmine blocks the active site of acetylcholinesterase, preventing acetylcholinesterase from breaking down acetylcholine molecules prior to reaching the postsynaptic membrane receptors.
