WRITING A NUTRITIONAL CARE PLAN I. Introduction

The nutrition care plan translates nutrition assessment data into a strategy that will address the nutritional and nutrition education needs of a client. Thorough nutritional assessment is vital in order to come up with an appropriate and sound nutritional care plan. It utilizes, summarizes, and correlates all gathered information about the patient including dietary, anthropometric, clinical, and biochemical assessment data. It also takes into account information on lifestyle, motivation and other personal factors.

II. Objectives

At the end of the exercise, the students were able to: 1. Describe each component of the nutritional care process; 2. Review nutritional assessment techniques; 3. Demonstrate skills in undertaking nutritional assessment; 4. Identify clinical problems that have nutritional implications from the data obtained from the patient; 5. Identify priorities for dietary management; 6. Enumerate objectives for the overall nutritional care of a patient; and 7. Write a nutritional care plan.

III. Activities

The students worked in pairs. They devised a nutritional care forms. Each interviewed their partner and got necessary information like personal data, clinical impressions, dietary recall and anthropometric data. After which, they assessed nutritional status of their partner and developed nutritional care plan. A sample one day menu was also prepared.
