I think this vision of OT for 2025 is well reflected at VAMC. In an acute care setting, it is our responsibility to assess patients’ needs in their current conditions and provide the best patient care as much as possible. We assist veterans where they should go next to increase and maintain independence; whether it is home, subacute rehab, long term acute care, or inpatient rehab potentially. We will discuss with other providers including social worker and medical provider about the patient’s home care needs after discharge from hospital. This includes need for home therapy, need for home health aides or Meals on Wheels, or need for an emergency life line system. In addition, we provide assessment and intervention, issues adaptive equipment
