Plan of Action
Each organization continues to find ways to improve patient care and decrease interruption, so patient flow management often has come under inspection. There are many articles regarding management of patient flow in patient admission and discharges. Often times organizations improve the patient flow by redesigning the processes system-wide that creates the flow problem. (Kirkbride, Floyd, Tate, & Wendler, 2012).

I will bring my articles to the meeting. Being a transformational leader, I would help lead the project. I would organize my ideas, take the issues and concerns to my clinical manager and Director of Nursing after collecting data and try to seek their support and attempt to set up a meeting.
Change Theory
The change theory/model I would explain the use of a Plan-Do-Study-Act (PDSA) cycle. The cycle is a four-method management method. It is often used to control and increase improvement. The PDSA is method is a learning process where knowledge occur through making change and reflecting on the consequences of that change (Kirkbride, Floyd, Tate, & Wendler, 2012). First, I would need to gather a team or committee of volunteers to help pilot the project. Members would be best if the team had the Chief Nursing Officer, the Clinical Manager and Clinical Coordinator of the team and a couple of nurses.
The committee would need to identify the peak admission times of patients, a group of admission team or admission nurse would need to assembled. An admission nurse or team would need to be available every day during the peak hours of the admissions. The bed coordinator’s job at our facility is to get as much information about the patient and determine what floor the patient belongs to. To get a patient admitted the bed coordinator would receive a call from a doctor or from the emergency department. An admission form is completed, and a call is made to the clinical coordinator/charge nurse of that floor. To keep track of all the admissions, the bed coordinator could document each admission received since they are the staff that places the patients to each floor. For the piloting program, the bed coordinator would notify the admissions nurse the same as they would notify a clinical coordinator. The admissions nurse would report immediately to the floor to initiate the admission. After a few admissions, the nurse could identify concerns and ask questions. If the admissions nurse is busy with one admission it would need to be communicated to the bed coordinator or clinical coordinator to start the admission to prevent
