Proposed Criteria to Conduct a Research Study on Backfill Assisted Voiding

Title and Description
a. Research Study for Backfill Assisted Voiding
b. Backfill assisted voiding is a procedure that is performed in the post anesthesia care unit. An indwelling catheter is placed preoperatively in selected patients in order for the procedure to take place. The “bladder was filled retrograde with room temperature sterile normal saline”. (Foster, RT Sr., Borawski, KM, pg. 627). Retrograde filling was stopped when the woman had a strong urge to void. The catheter was removed and the patient voided.
c. A cause for concern was initiated by the surgical outpatient unit because a MD is currently ordering the procedure on post-operative vaginal surgery patients with no standard practice. There is concern from members of the Nurse Practice Committee and Infection Control due to lack of evidence to support it and data to show the patient does not have negative consequences as a result of this procedure. Professional Outcome and Related Competency
a. The outcome of this proposal is to articulate the contributions of professional nursing to health care via research by defining the boundaries of nursing while gathering, organizing, planning, problem solving and reviewing in order to format an acceptable policy and procedure for standards of performance.
b. As a nurse we are held accountable for our actions. We must function within the Nurses Code of Ethics. We must engage in activities that promote evidence based practice such as familiarizing ourselves with gathering and monitoring data to support our proposal. The following guidelines are necessary for formulating standards of care for backfill assisted voiding: o Educational opportunities for staff and patients. o Minimum and maximum age requirements of participants. o History and physical of participants. o Risks and benefits of the procedure. o Cost. c. All these guidelines are relevant for the development of criteria evidence based practice which is defined as “The conscientious use of current best evidence in making decisions about patient care.” (Melnyk & Fineout-Overholt, 2005, p.6.)
d. A weekly meeting will be held after data from each trial becomes available. We will also meet with other interdisciplinary personnel to help formulate a plan of care for our clients.
e. This is a new procedure and not much data is available on this procedure. This poses a challenge for me to carefully use all the research skills I have acquired with the help of my mentor to emerge the desired client outcomes and show the relevance of this procedure in our practice.
Goals
a. To obtain crucial lessons on how to articulate and formulate a policy and procedure for standards of care through development planning consistent with nursing research. b. To stay abreast of current standards for risk associated with the proposed project.
c. Be aware of and adhere to all the ethical and moral research guidelines surrounding this research project.
d. Reviewing and tracking of all incident reports and litigations with this procedure. D. Strategic Plan
a. Meet with mentor to discuss the proposed project. {Time Frame, 2 hours}.
b. Plan on meeting with the other members of the Nurse Practice Council on proposed project once a week. {Time Frame, 1 hour/week; totaling 5 hours}.
c. Follow patient selected for trial of procedure from admission through discharge with mentor’s guidance. {Time Frame, 4 hours}.
d. Read literatures, search data bases and articles for information related to topic{Time Frame, 3 hours}. E. Feasibility
a. Based on ongoing review of research data, this plan provides for assessment, implementation and evaluation of data. All measures will be reviewed in the weekly meetings. This project will be completed using a set of tools from a variety of sources. One resource stated that, “Women undergoing transvaginal surgery are more likely to empty their bladder effectively before discharge if they are evaluated with a backfill assisted voiding trial. The role of my mentor is to guide me through the professional learning experience. She will accomplish this by meeting with me for at least once a week to evaluate the progress I made so far in the professional learning experience. She will also provide opportunities for professional growth, as it pertain to the professional learning experience.
