Age Well Senior services Program
History and background
As a result of increase in cases of uncatered health care issues for the ages in the county of orange, San Clemente seniors’ board, decided to come up with a solution. They voted in a board from the region. The board was supposed to carry out services in South Orange County. With time, it evolved to become a regional, non governmental, non profit organization. It continued to help the aged by fulfilling their needs and putting prevention programs. In early 2010 south county senior services decided to change their name to age well senior services.
Program description
Age well senior services program was developed with the sole purpose of helping the aged. It relies 100% on donations for finances to be able to provide services. Since 1975 when it was known as san clement seniors it has consistently provided vital services to the aged living in homes as well as all the aged low income earners in south county. These types of services are not easily available in other cities, the aging are always left to suffer on their own. They have a wide variety of services that they offer, they includes: * Meals on Wheels and Congregate Meal Programs * Adult Day Health Care and Alzheimer’s Social Day Care * Operation and Management of Senior Centers * Case Management * Non-Emergency Medical Transportation * Health & Wellness Programs
Meals on wheels and congregate meals programs
This program has been put in place to ensure that the aged never go hungry. They operate their nutrition program that ensures that seniors are fed well. They deliver food to the old Monday to Friday. They also deliver frozen foods for the weekends, thereby making sure they never have to go hungry. For the active seniors, they have feeding stations that serves hot and nutritious meals. This gives them a chance to socialize with their friends and talk about the current affairs. For them to be able to afford these meals daily they request $6.50 donations daily but no senior is left hungry for failing to pay. The aged in South Orange county request for meals on wheel services by calling the center location.
Adult Day Health Care and Alzheimer’s Social Day Care
Adult day health care is a licensed facility ran by age well senior services. It provides both day and health care services to the old. There are facilities for therapy since most of the seniors found here have disabilities. Recreational activities can also be found as well as social services. The seniors get to enjoy their stay in these facilities thereby giving respite to the people who care for them at home. The unit has professional staff, they include licensed clinicians in nursing, and different types of therapy. Transportation to and from the center is provided.
Operation and Management of Senior Centers
Senior centers are sources of vital services throughout South Orange County. They offer program that promote wellness of the seniors. Senior centers are also used as centers for meals on weekdays. There are professionals as well as volunteers who help in undertaking various tasks. Some costs for care are catered for by insurances and veteran administration.
Case Management
The old in the society are normally faced with various challenges that interfere with their normal living. This can be a huge problem for the aged and people around them. Age well senior services have put case managers in place to deal with these problems. The managers help the old to understand the options they have to be able to solve their problems and the steps to follow. Case managers are found in senior centers, and they can visit home if necessary arrangements are made. Case management enables the old to be able to reach professionals easily and to know who consult.

Non-Emergency Medical Transportation
This service ensures that seniors are provided with transportation for their various medical appointments that are not necessarily urgent. Attendants are supposed to pay $2 to ride and must be 60 years old and above. There is a limitation of eight trips a month but ongoing medical treatments are exempted. The riders have to register through the mail before they can start using the service.
Program goals
This program’s main goal is to take care of the old, emotionally and physically. This program ensures that no aged person in South Orange County is left uncatered for, by providing basic needs to them.
Clients and stakeholders
This program depends on well-wishers to fund them; currently there are a number of community partners who help to ensure they provide services. The old in South Orange County are the clients.
Logic Model
A logic model visually describes the inputs, action taken and output of a program. It describes how goals have been achieved. I have attached a copy of logic model for this program.
Reasons for the Evaluation
The main reason for evaluation is to know whether Age well senior services are effective to the aged in South Orange County.
