With approximately all health care organisations sharing in some kind of quality improvement (QI) activities (Ben-Tovim et al. 2008; Gowen Iii et al. 2008; Eller 2009), the absence of considerable improvements in quality is scary (Glasgow et al. 2010; Eller 2009). There is a need to seriously address the overuse of health services beside important efforts to reduce avoidable difficulties of care, that overuse has been nearly entirely left out of recent quality improvement activities (Chassin 2013).
