Family left the premises due to Domestic Violence situation with the father of the child in the composition. Family is currently residing at 976 Tinton Ave. Unit 46 Bronx, NY admission date 01/17/2016. Family composition consists of Raiza Machado (self, 29y); Miguel Cartagena (domestic partner, 25); Jorge Ortiz (Child, 9y); Nakia Ortiz (child, 8y) Mackenzy Ortiz (Child, 6y). Ms. Machado stated that no one will move with the family if permanent housing is obtained.
