On 7/26/2015 S/O EMT Perez was dispatched to OP502 regarding a pendant alarm. S/O EMT Perez knocked and announced his presence at the door. When there was no answer S/O EMT Perez open the door and announced his presence to which he was verbally invited into the bedroom by the resident, a Mrs. Carol Kulp. Mrs. Kulp stated that she fell in the kitchen when she lost her balance and she fell on her side and hit her head, she then crawled to the bedroom to retrieve and activate her pendant . Mrs. Kulp had a large hematoma above her right eye that had some blood around the wound but was not bleeding currently. S/O EMT Perez then immediatly suggested transport and described the possible consequences of delaying care with such a injury. Mrs. Kulp agreed
