On 8/25/16 at 1:55 p.m., in Secure I Dayroom, it was reported by FST, Charles Wheeler that Individual Donald Franklin was upset about rules regarding phone use on the unit. He became increasingly agitated, cursing and verbally threatening physical harm towards staff. Individual paced the day area with clenched fists intimidating staff and violently throwing furniture across the day area.
Code Yellow was called two times on individual. De-escalation techniques were ineffective. Wait strategy used to de-escalate individual. Individual was able to calm down and was in the process of walking with select FST's to outdoor fenced area to calm down. Individual began to exhibit sign and sitcom of a seizure that started. Staff removed all individuals from the area. …show more content…
Individual admitted not reporting to the previous shift because he felt ok at time of injury but as time progressed, he started feeling pain at the ankle area.
Individual Lowery is able to communicate his needs in a very clear tone and language but can be intrusive, non-compliant and needy.
08/25/2016 @ 6:05 p.m., in Secure III Dayroom, it was reported by FST, Ru’Shawn Thomas-Powell that Individual Audrey Williams felt that FST Powell was in her personal space, and got agitated cursing and making verbal threats. As FST Powell attempted to guard her face and body from harm. FST Powell and Individual Williams lost their balance falling to the floor. Individual got up and grabbed her shoes hitting FST Powell in my face again.
