Summary: Shannon Ragland is a 47-year-old female who was referred to MRS the St. Tarish Food Pantry. A comprehensive orientation was provided to Shannon in the office on 8/14/17. Shannon and counselor reviewed agency services; employment purpose of MRS program; that an employment outcome shall be in an integrated setting; eligibility criteria; services available through MRS and the statewide workforce investment system; the individualized nature of the program; the requirement to use other sources of funds before MRS can pay for services; how the applicant will be involved in making informed choices regarding their rehabilitation program; customer contribution; the agency’s order of selection for services policy; rights and responsibilities …show more content…
She still struggles with it, especially in the summer. Then in 1998, Shannon stated she was diagnosed with Lupus. Lupus is a chronic inflammatory disease that occurs when your body's immune system attacks your own tissues and organs. Shannon stated that her diagnosis affects her joints, walking, and getting out of bed. In 2003, she was diagnosed with rheumatoid arthritis in her legs and feet. This diagnosis causes her to have issues with standing for extended periods of time and walking long distance. Shannon’s most recent diagnoses were in 2011. She was diagnosed with a heart problem (blockage) and depression. Shannon is taking 11 different types of medications to treat the above diagnosis’s. A full list is in her case file. She does see several different doctor’s as well, at least once a month. Shannon signed releases for this counselor to send paperwork to her treating team to gather more information about her disabilities including her barriers/restrictions and if she is released to work competitively. Although she has barriers she indicated that her strengths include being a team player and hard worker.
Shannon stated some possible accommodations she may need on a job include: having a flexible schedule, having the ability to sit and stand when needed, be able to use her walker or cane if needed, and can use a head set so she doesn’t have to constantly pick up the phone.
Vocational Needs Assessment: Shannon mentioned that she has several physical limitations and barriers to working that only allow her to perform sedentary work. She does take medications and has a treating team for her disabilities. Counselor will be sending paperwork to her treating team to verify the above information as well as to determine if she is released to work. She will require vocational counseling that will focus on exploring if she can work competitively out in the community or would benefit from working from home. Shannon wants a job that will be flexible with her hours and were she doesn’t have to work nights or weekends. She also doesn’t want something where she has to stand or sit a lot.
