How can group practices be value-based and profitable?
Quality and value have historically been associated with profitability. This was the case in medicine as well. Those who were recognized as being quality providers of healthcare received increased business and subsequently increase profits. However, in today's competitive market a push for increased volume has often resulted in a inverse impacted quality. This is largely due to fee for service payment schedules which encourages providers to perform larger numbers of services and higher revenue generating procedures and tests. To push back against this government providers such as Medicare as well as other insurers are seeking new ways to provide reimbursement based on value rather than volume. This fear has been generated by the initiation of efforts such as the electronic medical record which at its initial inception was very cumbersome and resulted in providers being pulls away from patient care. Under fee for service as less patients were seen productivity decreased as did reimbursement. In attempt to counteract this trend that we see with fee for service, the government and other organizations have sought to change the way reimbursement distributed to providers. Value Based Purchasing in hospital and the use of meaningful use and now MACRAs in the outpatient setting strive to reward quality rather than quantity. The initiation of medical home groups and accountable care organizations (ACOs) are means by which providers can focused on quality care and remain profitable by taking advantages of changes made under the Affordable Care Act (ACA). The goal of manage care organizations like ACOs is rewards providers for performing better primary care and avoiding expensive and costly testing and hospitalizations.
