"I am the Petitioner in this case. I have had physical custody of the children for the past nine months. Respondent, Wysheka Shanet Collins has voluntarily relinquished the actual care, control, and possession of the children because about nine months ago due to a CPS investigation on her husband regarding abuse towards the children.
