Data Flow Diagrams

...5-3 Short Answer/Essay Questions: Data Flow Diagrams IT-510-X2879Advanced Info Technology 14TW2 1. Data flow diagrams show the flow of data through systems and are used during problem analysis. Systems do not transform input to a desired output in a single step. DFD's capture the information about the transformations and the movement of data through a system with bubbles and arrows to show relationships (Freetutes.) One of the four common mistakes often made when using data flow diagrams is an illegal data flow. All flows begin or end at a processing step, in which data is changed into a form of the final output. This way, mistakes can be identified and followed back to their source in order to be fixed. That is why it is important to ensure that there are no diagramming mistakes. The following are four descriptions of common diagramming mistakes that should be kept in mind when using a data flow diagram: A source or a sink cannot provide data to another source or sink without some processing occurring Data cannot move directly from a source to a data store without being processed Data cannot move directly from a data store to a sink without being processed Data cannot move directly from one data store to another without being processed (Whitten;Bentley;Barlow, 360.) Black holes, grey holes and miracles are a second class of mistakes when using data flow diagrams. These mistakes arise when the outputs from processing steps do not match their inputs....

Clients own documentation of the system: Some organizations have their on Standard Operating Procedures (SOP). These Standard Operating Procedures provides source of information and to the existing control systems. It is therefore important to check whether the system as described in the (SOP) is what actually practiced. c. Interview with staff: During various stages of the audit, the auditor will need to some members of the staff of the client and find out how they carry out the assigned duties. These questions will reveal existing control and give indication of potential problems. d. Now that we have a moment to pause, I think it would be best to re-evaluate our requirements gathering criteria. I took the liberty of analyzing our current documentation with Pat Nagaro, IT Project Manager, for inconsistencies or gaps. The inconsistencies found were: * Ambiguity and lack of understanding in what they were asked to do. * Workflow charts/diagrams are not included. * System requirements should be more defined. * Information should be stated in a clear and well formatted manner. Proposed New Approach * Meet with the work stream leads to evaluate current (Workshop 3) requirements gathering documentation. Making sure that the content includes action items, issue and risk registers. * Create flow charts of major processes that show the sequence of activity and the associated inputs and outputs. * Add any additional requirement if necessary. * Compile all the information together into one document. Clinical Flow Sheet and Problem List

...Clinical Flow Sheet and Problem List Nalini Patten HCIS/255 06/07/2015 Shawn Sledge Clinical Flow Sheet and Problem List Clinical flow sheet provides an electronic representation of the traditional paper flow sheet maintained during each inpatient stay patient Vitals Intake /Output. Wound Documentation, etc. are included into the work sheet. It presents data from multiply encounters into column form. This mean the clinician can view the information side by side so they can make comparisons of findings over a period of time. Flow sheet is housed in the patient's chart and serves as a reminder of care and a record of whether care expectations have been met (Gertee, 2012) Many clinicians think viewing a patient’s chart in the manner makes it much easier for them to spot trends in the patient’s health conditions. According to Gartee it makes it ideal for chronic disease management such as diabetes or long-term conditions such as pregnancy. OB clinic like using flow sheet to view their patient’s pregnancy it helps then view the patient’s previous visits while they are making documentation of current ones (Gertee, 2012). Before the EHR system was implemented clinician used paper flow sheet but EHR had the ability to be created dynamically. It also helps in preventive care flow sheet, clinician can see the date of the patient’s last mammogram and if the patient received the influenza vaccine this season. If all patient information is entered in fully and accurately, these...

Bussiness System and Deigns

...a Hospital Management System Data Flow Diagram A Data Flow Diagram (DFD) is a graphical representation of the "flow" of data through an information system, modeling its process aspects. DFD is often used as a preliminary step to create an overview of the system, which can later be elaborated. The purpose of data flow diagrams is to provide a semantic bridge between users and systems developers. The diagrams are: * graphical, eliminating thousands of words; * logical representations, modeling WHAT a system does, rather than physical models showing HOW it does it; * hierarchical, showing systems at any level of detail; and * jargonless, allowing user understanding and reviewing. The goal of data flow diagramming is to have a commonly understood model of a system. The diagrams are the basis of structured systems analysis. Data flow diagrams have the objective of avoiding the cost of: * user/developer misunderstanding of a system, resulting in a need to redo systems or in not using the system. * having to start documentation from scratch when the physical system changes since the logical system, WHAT gets done, often remains the same when technology changes. * systems inefficiencies because a system gets "computerized" before it gets "systematized". * being unable to evaluate system project boundaries or degree of automation, resulting in a  resulting in a project of inappropriate scope. Data Flow Diagram is denoted by using the symbols...

... Communications must be a two way free flow of information. The information exchanges occur between providers, staff, and patients or clients. This was an area that needed improvement was noted in the previous accreditation audit. Some noted prior issues from 2 years ago included patient and family education and information not being properly disseminated to the nursing staff. These are areas where we have targeted and currently meet. Some areas that we continue to work on are as follows. We currently need to address our time out policy. During the last year there were three months that Nightingale Community did poorly in this area. We must make sure that the time outs are not only conducted properly but more importantly documented in the patients chart. If the time out is not properly documented in the patients chart the organization will not receive credit, it will be as though it never occurred. We must make sure that all providers and clinical staff have appropriate training and training materials provided for the time out policy. We will continue to quantify our efforts monthly in this key area. We as an organization must make this goal monthly. Critical results are an issue of concern for the organization. Critical results must be reported within 60 minutes and this must be documented. This is an area like the time out policy where if it is not in the patient’s record it did not occur. We must strive to improve our documentation to support that our critical results have...

Effective Communication

...Effective Communication Introduction In a long-term care (LTC) nursing facility, the primary types of communication are documentation and verbal. The physicians and all of the nursing staff have to document every aspect of each patient’s care. Effective documentation provides a legal record of care and how the patient responds to the type of care which they received. By documenting, the physicians and nurses are giving written communication to those who provide follow up care. When the nursing staff begins their shift, the nurse gives a report of their previous shift’s activities. For example; the night nurse gives the day nurse reports on each patient. Without these verbal reports the nurses, Certified Nursing Assistants (CNAs) would need to read each individual patient’s chart to see what happened on the previous shift, this would take away valuable time from the patient and could lead to negative patient care. By using documentation this type of communication provides a legal record of care for each patient. The LTC facility uses several documents to monitor each resident’s care and the outcomes from the care. Each resident also has a personal medical record (PMR) these record contain the resident’s medical, family, social history, assessments, dietary needs, treatment plans, orders, prescriptions, progress notes, and lab results. With this chart the LTC has a complete and accurate look into each individual resident and their medical history and progression of care. Included...
