The term medical home has had quite the history! According to the PCPCC, Patient Centered Primary Care Collaborative, patient centered medical home is definitely not a new concept; even as far back as 1967, the American Academy of Pediatrics (AAP), actually introduced the term “medical home” (PCMH, 2015). Now, today’s term patient-centered medical home has evolved to define “a model of primary care excellence that is a patient-centered, comprehensive, coordinated, accessible, and committed to quality and safety” (PCMH, 2015). According to the government website, AHRQ, Agency for Healthcare Research and Qaulity, they define patient-centered medical home as a model which “holds promise as a way to improve health care in America by transforming how primary care is organized as well as delivered.” The AHRQ also states that the term medical home as not just a place, but also as a model of the “organization of primary care that delivers the core functions of primary health care” (PCPCC, 2016). When cultivating this type of medical home there are five functions and attributes that are involved: comprehensive care, patient centered, coordinated care, accessible services, and Quality and Safety (PCPCC, 2016).
Overall, it comes down to this: the primary goal of the patient-centered medical home, according to the AHRQ, is to improve the quality as well as the outcome of care, which is what patient’s are looking for. The medical world seems to always be looking for ways to make health care better or rather patient care. In 2006, according to the American Medical Association, AMA, two doctors conceived the term Accountable Care Organization (AMA, 2015). The ACO consists of a group of physicians, other health care professionals as well as hospitals who come together to correlate excellent care for Medicare patients (CMS, 2015). The goal of the ACO is to collaborate to make

The service is a complete healthcare service that is mobile and is dedicated to the treatment of diabetes in central and southeastern Ohio. Some of the services are provided by other pharmaceutical companies and some of the service is provided by current healthcare providers, but this service is educationally based and provides care to all diabetic patients. All patients will be accepted by the Mobile Healthcare Service (MHS). Private insurances will be accepted, managed care, and cash payers. But they must be referred to MHS by their primary care doctor or by the specialist such as an endocrinologist. First, I need to discuss how the service is set up and what the team will look like. The team will consist of a Nurse Practitioner, Doctor, Certified Diabetes Educator/Dietician, and Medical Assistant. This team will not be sponsored or run by the pharmaceutical companies and it will not be run by government money or grants. This service will be considered as a third party company providing additional service and education by going to the home of the patient. ...to benefit your Sparrow child in medical need. Service hours must benefit the public, neighbors or the local community. Youth, not adults, must earn service hours. Once you decide what community service you would like to perform, plan how it can be done – either as an individual or as a group. Make sure an adult supervisor signs the Sparrow community service vouchers verifying that you did the work, and how many hours it took. Students performing the service must know about their Sparrow and understand that the work they are performing is benefiting their Sparrow. Community Service is not fundraising. NEVER perform your Community Service without the permission of your parents or guardians. It is highly recommended that an adult supervisor be there when you are completing your Community Service. If appropriate, we encourage Clubs to perform “Acts of Kindness” for their Sparrow Family, as part of their Community Service. ...September 13, 2013 Mr. Homebuyer Prospect 144 Legal Lane Partnership, USA 3333 Dear Previous Customer: It has been quite some time since we last spoke. I wanted to catch up and give you an update as to what is happening within the world of real estate and mortgages. Certainly a lot has happened nationally since I last financed your home. We have gone through a real estate boom and a recession and home prices have fallen from their peaks a few years ago. The recession has provided pain for many Americans. It also has provided opportunities for many others. With regard to real estate, lower home prices have translated into bargains. Many investors are purchasing foreclosures and other distressed properties. Others are taking the opportunity of the lowest rates in history to refinance. I have refinanced many previous clients who have achieved hundreds of dollars in monthly savings and others who have moved into 15-year mortgages. With homes not appreciating as they were, building up equity through prepayment is a very popular objective today. Some have lowered their rate enough that they have been able to move to a 15-year schedule without increasing their payment! I would like to schedule a phone call with you to get an update as to what is happening in your life and financial situation. I may be able to give you an idea of what values are in your neighborhood and how your present credit score can be improved to further your future financial prospects. Please...

...Life and School: Making it work Education is crucial in today’s world, and most times there is a need to choose to either work full time or go to school full time. In most cases, choosing one or the other is not an option. Finding the time to balance college classes with all of the activities in your daily life is what most people who are considering going back to school find most difficult and discouraging. However, with the right strategy balancing life and school can be manageable. We live in an age where women play a crucial role in the job market and in the home setting. This is no exception for me. Prior to school, my work and home life consisted of being a mother to my dogs, a wife to my partner, a leader and manager at my job, a socialite to my friends, a dedicated family member, a maid and a cook! Trying to factor in college classes seemed frightening at first! It has been ten years since I have attended any kind of educational classes; however, seeing that I regretted not attending college directly after high school, I made the decision that going back to school would open up many career opportunities for me. Deciding that I wanted to go back to school to expand my knowledge and career opportunities was the biggest decision that I have ever made thus far. Since I do not have the luxury to always have a set schedule to where I can attend traditional college classes in a classroom, I made the big decision to do distance learning as opposed to traditional...

We are going to analyse the difficulties in expansion in the U.S. market and use a SWOT analysis to assess the circumstances that affect the same. In addition, we would give our proposed strategy and the three-year plan both in U.S and Indian market. IKEA’s expansion plan in U.S, faced a lot of problems. Firstly, the economic recession had a deep impact on the home furniture industry in America. For example, the home furnishings sector decline strongly in 2008 by -7.9% to 94.58 billion dollars. Secondly, the demand for customization and convenience is strong in U.S. Because unlike people in other countries, the U.S. people prefer to store most of their clothes folded while Italians like to hang. They were buying vases to drink because of the too small glasses. In short, they want more comfortable sofas, higher-quality textiles, bigger glasses and more spacious entertainment units. ...Segmentation The following categories represent the people who would be the most likely to adopt the behaviour because they do not live a single life by themselves. They share the space at home with at least one person on the daily basis. Segment 1: Four nurses who are married/ in couple and do not have children These four employees would be the most difficult to convince because they could argue that because they do not have a child to take care of once they get home, they should not have to wash their hands as frequently as their colleagues who are parents. These nurses could think that there would be no serious consequence if they allow themselves to be a little careless. When confronted with the possibility of contaminating their partner or husband, they could well say that because their husband or partner works late, they fall asleep by the time he finally comes home. They could also say that because they are both adults in the household, each person should know how to take care of oneself. Or else, these nurses could argue that the risk for grown people to be contaminated by a sick spouse or partner’s germs is barely existant in comparison with households where a child is present. Segment 2: Thirty three nurses who are married/ in couple with children Segment 3: Five nurses who have children but are not married/ in couple These two categories representing a total of thirty eight people could actually be put together because there is a child factor. Laundry Research

...INTRODUCTION In today's busy world, balancing priorities is quite challenging. The twenty four hours seems isn’t enough to finish all the demands of the day. This dilemma calls for time management. An individual must have time for work, for a break, and for boundless tasks at home. One of these never ending household chores is laundry. Historical dates show that laundry was originally not done at home. People in the ancient times did their laundry in streams and rivers. As time goes by, laundry was moved in-house and there have been improvements on how people wash their clothes. Washing machines were then invented, and these machines make laundry more convenient and at home. But as peoples’ life change in response to the fast growing technology, many of them haven't have the time to do their laundry, since it is still time-consuming even with the help of machines. Most people think that the time for laundry can be allotted to studies and in the advancement of their careers. So this is why, laundry shops were addressed. Many people use it as their business in order for them to have an income. Before technology has began, there are some early laundry methods and tools like washing clothes in the river, Laundry shops were also called as a Laundromat, where it is a type of launderette, used for a place that has machines for washing and drying clothes, towels, sheets, etc. Some laundry shops, the owner or employees were the one who do the service.
