This report will particularly focus on some of the recent studies into Thalidomide’s teratogenic mode of action on malignant MM cells; specifically its mode of action on cereblon (CRBN) in MM. Results Studies have revealed that Thalidomide directly binds with CRBN, a protein that forms a complex with E-3 ubiquitin ligase activity along with its binding partners, cullin-4A (CUL4A), DNA-binding protein-1(DBP1) and regulator of cullins-1(ROC1). When Thalidomide binds to CRBN, it is thought that CRBN recruits and binds substrate proteins in this complex resulting in the ubiquitin-proteasome pathway of the proteins, Ikaros (IKZF1) and Aiolos (IKZF3). These transcription factors are essential for B cell, T cell and plasma cell development. Supporting this theory, Thalidomide-resistant MM patient cells were analysed retrospectively. Results showed that patients had suppressed levels of CRBN, and thus suppressed levels of IKF1 and IKF3 in myeloma cells.
